1 INTRODUCTION Inservice Inspection Examinations of ASME Class 1,2, and 3 piping welds and components were conducted at Seabrook Station in accordance with ASME Section XI,1983 Edition through Summer 1983 Addenda and the ISI Program Plan. This report summarizes ISI examinations completed during the third refueling outage encompassing the period from April 9,1994 through July 25,1994. ISI Period 1 is complete with approximately 34% of the Interval 1 examinations completed.

SUMMARY

1.0 NONDESTRUCTIVE EXAMINATION PROCEDURES The following procedures were used during refueling outage #3 inservice inspection.

Four procedures utilized (VT Vessel Visual, PT, & MT) were North Atlantic Energy Service Corporation (NAESCO) procedures. The remaining procedures used were Yankee Atomic Electric Company (YAEC) procedures approved for use by NAESCO Station Operation Review Committee (SORC). YAEC serves as the certifying agent in NDE for NAESCO.

SUMMARY

REPOI{I The following is a summary of all examinations performed, conditions noted, and corrective measures taken during the third refueling inservice inspection.

Code Category B-A Pressure Retaining Welds in Reactor Vessel The following examinations were performed on the reactor vessel head. Two thirds of these welds were examined from the centerline of stud hole #50 clockwise to the centerline of stud hole #33. This completes the examination on these two welds as one third was conducted during the first refueling outage.

RC RPV 103-101 UT examination was perfonned on this upper head circumferential weld with no recordable indications. Scan limitations and geometric reflectors from CRD penetrations were verified by previous outage and preservice data.

RC RPV 101-101 UT and MT examinations were performed on this head-to-flange weld with no recordable indications. UT scan limitations and geometric reflectors from CIU) penetrations were verified by previous outage and preservice data. Magnetic particle examination was performed with no unacceptable indications.

Code Cateeorv B-F Pressure Retaining Dissimilar Metal Welds RC E-10 A-SE Ultrasonic examination revealed an indication at varying amplitudes for 360* around the safe end. Evaluation determined that the indication was caused by beam redirection due to columnar grain structure of the weld metal. Full ASME Code coverage was not achievable. Calculated coverage for this weld was 63%. Relief will be pursued. Liquid penetrant examination was conducted during the first refueling outage with no unacceptable indications.

RC E-10 B-SE Ultrasonic and liquid penetrant examinations were performed with no unacceptable indications. Full ASME Code UT coverage was not achievable. Calculated coverage for this weld was 56%. Relief will be pursued.

RC E-10 C-SE Ultrasonic examination revealed an indication at varying amplitudes fbr 360' around the safe end. Evaluation determined that the  ;

indication was caused by beam redirection due to columnar grain ,

structure of the weld metal. Full ASME Code coverage was not j achievable. Calculated coverage for this weld was 71%. Relief will l be pursued. Liquid penetrant examination was performed with no unacceptable indications.

RC E-10 S-SE Ultrasonic and liquid penetrant examinations were perfomled with no unacceptable indications. Full ASME Code UT coverage was achieved on this weld.

RC E-10 SP-SE Ultrasonic examination recorded two indications at varying amplitudes for 360* around the safe end. Indication #1 was a geometric reDector from the ID which was seen with the 27' shear wave component of the 60* refracted L-wave. Indication #2 was also a geometric reflector from the ID which was seen with the 22* shear wave component of the 45' refracted L-wave. Both were verified with an ID profile and beam plot. Full ASME Code coverage was not achievable. Calculate covetage for this weld was 61%. Relief will be pursued. The above tests successfully underwent a VT-2 visual examination. With the exception of minor packing and flange leaks at test pressure, the inspections were completed with no leakage present. The minor packing and flange leaks were corrected via the station work I control program and satisfactorily retested.

14. ' Abstract of Results of Examinations and Tests ASME Class 1 B-A Two thirds of two reactor vessel head circumferential welds were examined. This completes these two welds as one third of their area was examined during the first refueling outage.

B-F All six Pressurizer safe ends were examined. Full ASME Code UT coverage could not be achieved on five of the six welds. Relief will be pursued. Three of the welds exhibited beam redirection due to columnar grain structure of the weld metal.

One safe end weld had two indict.tions which were evaluated as geometric reflection from the ID.

ASME Class NF Several component supports were identified as having minor problems (i.e. clearances, gaps, etc.). Evaluations were performed and the supports were detennined operable, and did not fall within IWF-3410.

15. Abstract of Corrective Measures Welds and Bolted Comnonents No corrective measures were required as a result of UT examinations. All recorded indications were evaluated and deermined to be related to geometry and weld metal structure. Surface and visual examinations also did not warrant corrective measures.

k Pressure Testing .

No corrective measures were required as a result of pressure testing. Minor packing and  ;

flange leakage was corrected via the station work control program.

Component Sunnorts No corrective measures were required as a result of component support visual examinations. Clearance and gap discrepancies were evaluated by Engineering as acceptable.
