t 3.0 Introduction I

In-August of 1991, steam Generators A, 3, C,

and D underwent their first inservice inspections.

This was accomplished in accordance with Seabrook Station Technical-Specification 4.4.5.

This report presents the results of the inspection pursuant to Technical specification 4.4.5.5.b.

The following results are presented:

e.

The number and extent of tubes inspacted.

The location and parcent of wall-thickness penetration for each e

-indication ol'an imperfection.

Identification of tubes plugged.

Seabrook Station _is a Westinghouse four loop pressurized water reactor with Hodel F steam generators.

The generators are U-bend heat exchangers with tube bundles fabricated using thermally treated Inconel:600 tubing. Each tube is identified by a row and column numbor; I

there are 59 rowv and 122 columns in each uteam generator for a total of 5,615 tubes.

Nominal tube OD is 0.688" with a 0.040" nominal wall.

The Technical Specifications require a tube to be plugged when tube wsil-loss equale or exceeds 40% of nominal wall.

No tubes were required t

to be plugged as a result of eddy current inspection. However, a total _

of 12 tuces in all four gerarators were preventively plugged.

Bobbin test data was recorded at tour frequencies in both differential and absolute modes.

MRPC data was recorded at four frequencies in the absolute mode only.

All test data was recorded onto digital tapes and subsequently onto optical disks for analysis.

tidy current data was independently analyzed by primary and recondary reviewers supplied by CONAM Nuclear, Inc.

Any discropancios were resolved by the analyst shift supervisor and the Yankee Nuclear Servicco Division (YNSO) Eddy Current Level III.

Analysts were trained on site to the Seabrook Station Steam Generator Eddy Curront Analysis Guidelines and woro given a practical examination consisting of typical Model F indications on three reels of sample data.

The YNSD Eddy Current Lovel III perf or ned surveillance on the contractor, reviewed reuults of inspe tions, and resolved discrepancies r

between primary and secondary analysit resulto in conjunction with the CONAM lead analyst.

The Lovel III also reviewed all procedures, guidelines, and training programs prior to inspections.

through 4'.

Using the Technical Specification critoria, the flaws can be categorized as follows:

Eddy current indications below 20s wall-loss, if detectable may be considered as imperfectionu.-

Imperfections can be divided into two groups (1) relevant wall-loss signals of flaws and-(2) other relevant indications.

The number of tubes _with indications found as a result of the eddy current examination are summarized below. The number of tubes, as listed below, does not necessarily reflect the number of indications, eg. multiple indications may be found in a single tube.

s Thero wSre a significant number of dents in Steam Generators D and C.

It should be noted that these dents were identified in the preservice examination.

Of the above groups of indications, the only one that appears to be service related is the group of tubes with AVB wear. AVB wear in the Seabrook steam generators is consistent with other plants' experience with Model F steam generators.

The preventive plugs were installed for neveral different reasons. In Steam Generator.B, two of the three tubes were bare hole plugs which were replugged.

The third was a large manufacturing burnishing mark that was. called 37% Traugh-wall.

In Steam Conorator C.

four of the eight tubes were plugged to bound a loose part that could not be retrieved. Three of the-remaining four were Ava wear called 36%, 36%,

and.37% through-wall. The_oighth was a low amplitude (0.4 volt) signal' I

that was called 38% through-wall. In Steam Generator D, one AVB worn tube.was. plugged at 35%;throughwall.

No plugs vere installed in Steam Generator A.

CONCLUSIQH l

.The inspection of the four Seabrook steam generators revealed nervice related degradation from antivibration bar wear.

This data is consintent with industry Model F experience and EPRI identification of AVD-wear as the.only active. mechanism in Model F steam generators. No action relative to moderating this condition can be-recommended.

No

' tubes exceeded the Technical specification plugging limits.,-however-12 tubes were preventively plugged.
