Dear Sirs:

The Idaho State University AGN-201 Reactor Facility (License # R-110, Docket # 50-284) is submitting corrections to the Report of Self-Cited Violations of July 14,1994. Details of the Report of Self-Cited Violations of July 14,1994 discussed how there was a failure to record radiation and conege or contamination surveys on four separate occasions.

Engiering

$[cio"$$# On July 15,1994, two of the four missing radiation and contamination 83m8&o survey records were discovered. The survey records of November 10 and 11,1993 were found in the Reactor Health Physics Log.

The resultant discovery of radiation and contamination survey records does not alter in any way the corrective actions to be taken, the date of full compliance (September 30,1994) to correct the self-cited violation of July 14,1994, or the notification the USNRC will receive after full compliance has been achieved.
