REPORT OF VIOLATION DISCOVERED DURING THE USNRC JULY 1994 INSPECTION Hjstory of the Violation On July 20,1994, during an inspection of the Idaho State University (ISU) AGN-201 Reactor Facility by the USNRC, a violation of Technical Specification 4.2.d was discovered. Technical Specification 4.2.d requires that a channel test of the seismic displacement interlock be performed semiannually. However, it was discovered that more than six months had passed between the last two times that this test had been performed. The test had been successfully completed on October 29,1993 and June 16,1994.

While the Reactor Supervisor was reviewing the ISU AGN-201/SNM Tracking Chart with the USNRC inspector, the violation was identified. The seismic displacement interlock channel test performed on October 29,1993 was crossed out as completed on the tracking chart, but not scheduled for the next occurrence when this test was to be performed. The channel test was performed again during the seismic displacement interlock channel calibration conducted on June 16,1994.

Cause of the Violation The Reactor Supervisor was the only person on the reactor facility staff required to track periodic facility requirements. (There are approximately seventy-seven separate periodic facility requirements tracked.) The Reactor Supervisor failed to schedule the seismic displacement channel test for its next occurrence. The root cause of this violation is human error.

Corrective Actions to be taken to orevent a violation of this nature from re-occurring The Reactor Administrator and the Reactor Supervisor will both perform separate and independent reviews of the tracking chart on a monthly basis. The review will generally occur near the beginning of the third week of each month. Upon verifying that the tracking chart is up-to-date, the Reactor Administrator and the Reactor Supervisor will both independently sign and date the tracking chart thus indicating that their review is complete for the month.

The Reactor Administrator will be trained on how the tracking system functions by the Reactor Supervisor. Monthly reviews of the tracking chart by Reactor Administrator and the Reactor Supervisor will begin starting July of 1994.
