SUMMARY

OF THE OPERATING REACTOR EVENTS MEETING ON OCTOBER 7,1981 On October 7,1981, an Operating Reactor Event meeting was held to brief the Office Director, the Division Directors, and the'ir represen-tatives on events which occurred since the last meeting. The list of meeting attendees is included as Attachment 1.

The events discussed and the significant elements of these events are presented in Attachment 2.

In addition to these events, the assignment of follow-up review responsibility was discussed. The assignments made during this meeting and the status of previous assignments are presented in Attachment 3.

The next Ope, rat.ing Reactor Events meeting is scheduled for October 14, 1981.

The following Notifications of Significant Enforcement Action were dispatched during the past week:

a.

EN-81-33 Union Electric Company - A Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $2,000 was issued to subject licensee on September 25, 1981.

This action was based on alleged violations relating to the failure to follow procedures to lock out level gauges, which contained radioactive material, before the li.censee's ' personnel performed maintenance in the coal hoppers in which the gauges were instalki.

This resulted in an individual receiving a calculated 3.5 rem whole. body exposure.

b.

EN-81-34 Notices of Violation were forwarded on September 22,1981, to six licensees for their failure to inform NRC prior to July 1,1981 of their ina-bility to complete implementation of the prompt public notification system.

This system was required to be operational by July 1,1981 in accordance with the revised emergency planning rule of 10 CFR 50.54(S)(2). This rule requires each nuclear power reactor licensee to demonstrate that administrative and physical means have been established for alerting and providing prompt instructions to the public within the plume exposure pathway of the Emergency Planning Zone within about 15 minutes.

The Notices of Violation were sent tc:

Dairyland Power Cooperative-Lacrosse, Consumers Power Company - Palisades, Tennessee Valley Authority - Browns Ferry, Units 1, 2, and 3. Wisconsin Public Service Corporation - Kewaunee Iowa Electric Light and Power -

Duane Arnold, and Jersey Center Power & Light Company - Oyster Creek.

EN-81-35 Carolina Power and Light Company (Brunswick Unit 2) - A Notice of c.

Violation and Proposed Imposition of Civil Penalties in the amount of $40,000 was issued to subject licensee on October 1,1981.

This action was based on alleged violations involving failure to perform an adequate radiation survey that resulted in an exposure to an individual in excess of NRC limits and the use of a technically unqualified radiation control technician that was a causative factor to the excessive radiation exoosure.

EH-81-36 Nuclear Diagnostics - A Notice of Violation and Proposed Imposition '

of Civil Penalties in the amount of $1,000 will be issued on or about October 6,1981, to subject licensee.

This action is based on alleged violations involving failure to perform. adequate surveys that resulted in an uptake by an employee into the body of iodine-125 in excess of NRC limits.

Three other violations not related to the excessive internal exposure occurrence were also found.

The nature and significance of those items were such that civil penalties would not be warranted.

The following Notifications of Significant Enforfement Action were dispatched during the past week:

EN-81-31 Tennessee Valley Authority, Sequoyah Unit 2 - A Notice of Violation and a Notice of Proposed Imposition of Civil Penalties in the amount of $40,000 was issued to subject licensee on September 24,1981. This action was based on apparent items of noncompliance at the Sequoyah fapility which resulted in. exceeding a Technical Specification Limit 4ng Condition for Operation.

The imediate cause of the event was a failure to return three recirculation valves in the containment spray system to their nomally locked-shut position, as required by procedure.

This failure rendered a system designed to prevent or mitigate a serious safety event unable to perform its intended function.

b.

EN-81-32 Public Service Electric and Gas Company, Salem Nuclear Generating Station, Units 1 and 2 - A Notice of Violation and a Notice of Proposed Imposition of Civil Penalties in the amount of $40,000 was issued on September 24, 1981 to subject licensee.

This action was based on apparent items of noncompliance at the Salem facility which denonstrated inadequacies in the management of the licensee's physical security program.

These itens concerned the abuse of the. licensee's photobadge system by the site security force.

Vogtle construction site - On 9/23/81, while making a concrete pour in a confined area,11 workers were overcome by carbon monoxide fumes produced by a gasoline powered vibrator.

Four of the workers required continued monitoring at a local hospital.

No serious injuries were sustained.

Activities of the Divisio'n of Emergency Preparedness:

September 23, 1981 Deputy Director and Chief, IRB, participated at the NRC State Liaison Officer's meeting.

Staff member from EPLB assisted a FEMA Region 3 representative with development of testimony for the Susquehanna hearings.

Staff member from EPLB participated in the Summer hearing.

September 17, 1981 A meeting of headquarters and regional Emergency Preparedness review teams was held on 9/17/81 to discuss means of expediting onsite emergency preparedness i

appraisals and reports. As a result of this meeting, report length will be reduced and report review proceoures improved.

Schedules for the renaining site visits and personnel assigned to the teams will be: stabilized and consultation on schedule changes improved to reduce stress on tearileaders and team members.

To date, 21 site visits have been completed and 5 reports issued.
