Attachment i contains information regarding radioactive effluent releases. Attachment II contains information regarding solid radioactive waste. Attachment III contains inforn tion regarding the inoperability of certain effluent monitors during the toporting period. Attachment IV contains Revisions 5 and 6 to the Catawba Specific PCP. Control Copies 26-31 have been sent to the NRC Document Control Desk. Control Copy 22 of the revisions has been sent to Region II. Description:
Unit 1 FWST water spilled inside the Restricted Area via a temporary sample hose hooked to the tank vent for { sampling purposes. The sample hose was inserted 4 to 8 .) 0 feet into the tank _ vent _on top of the FWST. This length -l was longer than needed to collect a sample. The spill l and subsequent siphoning effect contaminated approximately 650 sq. ft. outside the RCA, Cause: The cause of the incident was human error. The' ;j technician placing the tubing into the FWST did not j measure the length of the tubing and did not realice the j impact of placing excess tubing into the tank. ! l Sample hose was removed from the FWST Vent (Units 1 & Corrective Actions: 2). Approximately 1500 cu ft. of contaminatad soil.was- j' removed from the spill site. Temporary hosing was set :J up to pump FWST sump water to the Steam Generator drain j tank. Samples were analyzed to determine the source of 1 the contamination. A step was added to procedure HP/0/B/1000/02 on 6/27/90 to require RP supervision to observe and approve abnormal sampling of tanks, piping, , etc. .that are not specifically addressed by the j procedure. t Consequences: There were no Dose Calculations performed for this_ :

Description:
Opening of valve ND32A with ND Train A operating and ND33 full open caused NC System water to be transferred to the FWST. An estimated 5000 to 6000 gallons were received'by the FWST in 30 seconds. Lost Pressurizer 3 level and reduction in NC system pressure required tripping of NC pumps 1B and 1D. Diversion of ND flow i from the loops allowed NC system temperature to go above 200 degrees F. Cause: An error in valve alignment resulted in the inadvertent transfer of the NC water to the FWST. s Correcti.a Actions: Performance /MES to determine the affect on ND Pump 1A. ! MES to determine the affect _ on the NC pumps.- Design Engineering completed an Operability Evaluation on June 15, 1990 which concluded that the portions of NC,. . ND and FW exposed to this event were not damaged and l that they perform properly, therefore remaining operable. These conclusions were also supported by evaluations performed by Westinghouse (Letter ; MT-SMDT-279(90), dated 6/14/90). A. sampling program .j was set up to track the concentrations of gases every 6-hours that are at the top of-the tank. Consequences: A total of 3.847 Curies of noble gas were available for l release from the FWST. Based on the sampling program, l an estimate of 1 Curie was released.
