OF UNPLANNED RADIOACTIVE RELEASES TO UNRESTRICTED AREAS CATAWBA NUCLEAR STATION e
Event Description On April 12, 1988 approximate 1,, 62.6 cubic feet of radioactive gas leaked from Shutdown Waste Gas Decay Tank (SWGDT) A into the Aux Building atmosphere while work was being initiated on valve IWG235.
Cause This release was caused by the failure of Waste Gas (WG) Drain Trap T-03 while work was being initiated on valve IWG235. Route of Release:
SWGDT A --> G Compressor B --> WG Trap T-03 (trap failed) -->
Waste Evaporator Feed Tank (WEFT) Sump B --> Room 214 Aux Building Corructive Action There is a design deficiency which causes leakby on WG drain traps. Nuclear Station Modification (NSM) - CN50382 was written to replace a similar drain trap WG T-04 with a receiver tank and change the controller for valve IWG33Q to a 3-way switch. Drain traps WG T-01, WG T-02, and WG T-03 will be added to the NSM for replacement by station Chemistry.
Consequences Approximately 62.6 cubic feet of radioactive gas leaked into the Aux Building.
Total noble gases released was 1.03E-01 curies. Total particulate released was 3.49E-05 curies.
