CN-702.15 Dear Mr. Faris This letter is to provide you written follow-up of our telephone conversation Thursday, September 8,1994. As we discussed, visible foam was noticed in our discharge at Outfall 001 at approximately 1215. Looking int'o the source of the foam it was discovered that a liquid release from the Waste Monitor Tank in the Liquid Radwaste System was in progress. This tank contained water from the site laundry and was being released through Outfall 004 which is an internal outfall connecting to our once through non-contact cooling water and discharging through Outfall 001.
This release was secured at 1225 and the foaming at Outfall 001 ceased within minutes of this action. The accumulated foam quickly dissipated and a visual check of the discharge cove was performed.
Notification was made to you at 1300 and the Nuclear Regulatory Commission was~also made aware of the event. As required by our NPDES permit we will include reporting of this incident with our Discharge Monitor Report for the month of September.
A formal Problem Investigation Report (PIP) has been initiated to investigate this incident, determine root cause and implement corrective actions to prevent recurrence.
