10) EVENT DESCRIPTION AND PROBABLE CONSEQUENCES-WHILE PERFORMING ROUTINE CONTROL BOARD CHECKS OPERATOR FOUND THE CONTROL SWITCH FOR THE CONTAINMENT PRESSURE EQUALIZATION (CEQ) FAN IN STOP AND THE BACKDRAFT DAMPER FOR THE OPPOSITE FAN WAS OPEN.

THE FAN WOULD NOT HAVE STARTED AUTOMATICALLY 10 MINUTES AFTER RECEIPT OF A CONTAINMENT ISOLATION PHASE B SIGNAL. THE ABILITY TO MANUALLY START THE FAN WAS NOT IMPAIRED. THE ACTION STATEMENT OF T.S. 3.6.5.6 ALLOWS THIS CONDITION TO EXIST FOR 72 HOURS IN MODES 1, 2, 3 AND 4. THIS TIME LIMIT WAS NOT EXCEEDED, HOWEVER, THE REQUIREMENT OF T.S. 3.0.4 " APPLICABILITY" NOT TO RELY UPON PROVISIONS OF THE ACTION STATEMENT TO CHANGE MODES WAS EXCEEDED AS MODE 4 WAS ENTERED WITH THE SWITCH IN THE STOP POSITION. THE OPERABILITY OF THE OPPOSITE FAN WAS NOT AFFECTED BY ITS DAMPER BEING IN THE OPEN POSITION AS IT WOULD HAVE RETURNED TO THE CLOSED POSITION IF IT WAS EXPOSED TO A BACKPRESSURE, AS WAS SUBSEQUENTLY VERIFIED THROUGH USE OF ITS TEST SWITCH.
