'kTTACHMENT TO LER#82-015/04L-0 SUPPLEMENT TO CAUSE DESCRIPTION THE CAUSE OF THE FLOW INDICATION FAILURE IS UNKNOWN SINCE THE PROBE IS A SEALED UNIT: HOWEVER, M0ISTURE IN THE SYSTEM IS BELIEVED TO HAVE BEEN A CONTRIBUTING FACTOR. PRESENTLY, A DESIGN STUDY IS IN PROGRESS TO REDUCE THE M0ISTURE PROBLEM IN THIS AREA. DURING REPAIR AND INVESTIGATION OF THE PROBE PROBLEM, IT WAS DISCOVERED THAT REPLACEMENT PROBES HAVE BEEN CALIBRATED TO A RANGE OF 0 TO 200 FEET PER SECOND (FPS) BY THE MANUFACTURER (THERM 0-SYSTEM INC.) RATHER THAN IN CUBIC FEET PER MINUTE (CFM). FURTHER INVESTIGATION REVEALED THAT ALL PROBES USED IN THE PAST FOR THE REPAIR OF THE FLOW INDICATOR HAVE APPARENTLY BEEN OF THE SAME FPS CALIBRATION. THE DIFFERENCE BETWEEN THE FPS AND CFM CALIBRATION HAS MADE VARIOUS ELEMENTS OF THE FLOW MEASURING CIRCUIT INCOMPATIBLE. T0 RESOLVE THIS PROBLEM AN INTERIM SYSTEM FOR MEASURING FLOW HAS BEEN INSTALLED AND WILL BE UTILIZED UNTIL A PERMANENT SYSTEM CAN BE PLACED IN OPERATION. THE NEW SYSTEM WILL BE INSTALLED PENDING THE RECEIPT OF EQUIPMENT FROM THE SUPPLIER.

SINCE THE PROBLEM WAS DISCOVERED, A REVIEW 0F RADI0 ACTIVE EFFLUENT REPORTS HAS BEEN PERFORMED. THIS REVIEW INDICATED THAT THE FLOW MEASUREMENT WAS NOT REQUIRED FOR QUANTIFICATION OF ANY RELEASE AND THEREFORE DOES NOT AFFECT ANY ROUTINE REPORTING.
