Description:

During normal operation, snubber GBB-3-SS-235 on the "B" RHR system was removed from service for testing. The "B" RHR system was declared inoperable while this snubber was out of service. With the "B" RHR system inoperable, the "B" containment spray system was also effectively made inoperable. Due to the "A" RHR (LPCI) syttem having previously been declared inoperable (see R0 82-014), a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> LC0 was entered per Technical Specification 3.5.A.6.

This event was initially classified as a 30 Day Reportable 0ccurrence but was reevaluated as a 14 Day Reportable Occurrence on March 1, 1982.

Cause

Description:

The snubber was removed for testing after the pipe clamp to which it is attached was found rotated approximately 30 degrees from its proper position and the snubber extension was found bent slightly. The cause for these two problems is unknown. These problems in themselves did not effect the system operability. The snubber was removed to verify that it was operable. The snubber is a Bergan Patterson Model HSSA-10 hydraulic snubber.

Corrective Action:

The pipe clamp was repositioned and secured, the extension piece was r

straightened, and the snubber replaced with a like-for-like spare. The snubber was out of service for approximately 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />. The original snubber was tested with satisfactory results.
