_r* Licensee Event Report - Supplemental Data Docket No. 050-0331 Licensee Event Report Date: 2/19/82 Reportable Occurrence No.: 82-009 Event Description During normal operation while moving new fuel through the opened outer reactor building railroad airlock door, the inner railroad airlock personnel door was inadvertently opened, violating secondary containment integrity. This violated Technical Specification 3.7.c. The personnel door was immediately closed to restore secondary containment integrity and a security watchman was temporarily posted at the door to prevent recurrence. Secondary containment inte was promptly verified by testing. One previous occurrence (R0 76-15)grity Cause Description The cause was personnel error with a contributing cause of a failed solenoid in the door interlock mechanism. An indicating light was operable and indicating that the outer door was open. The person who opened the door apparently ignored this indication.

Corrective Action Site personnel will be instructed concerning secondary containment requirements.

The defective solenoid was replaced. The interlock design will be reviewed in an effort to increase reliability.
