Description:

During normal. operation while performing surveillance testing, the "A" standby filter unit (SFU) for the control building ventilation system was declared inoperable when a halogenated hydrocarbon test indicated a charcoal absorber bank removal efficiency of 97%. Technical Specification (TS) 3.10.A.2.a requires a halogenated hydrocarbon removal efficiency of >/= 99%. A 7-day limiting condition for operation (LCO) was entered as required by Technical Specification 3.10.A.3 for an inoperable SFU train. The low removal efficiency was believed to be caused by moisture in the "A" SFU charcoal absorber bank. While the declared-inoperable "A" SFU was being operated to dry out the charcoal absorber bank, the operability test of the "B" SFU was begun. The "B" SFU performed as designad by not auto-starting while the redundant SFU train was running. When this feature of SFU operation was encountered, the "B" SFU was manually started and operated during the remainder of the "A" SFU drying-out period. As defined by Technical Specification 3.10.A.4, a 24-hour LC0 was briefly entered during the period that the declared-inoperable "A" SFU was running before the "B" SFU was manually started. During this period, the "A" SFU was not running with the designed filtration efficiency and the "B" SFU would not have auto-started upon demand. The "A"SFU was made operable approximately I day into the 7-day LCO.

There have been no previous similar occurrences.

Cause

Description:

The cause of the low filtration efficiency of the "A" SFU charcoal absorber bank ,

was the collection of moisture in the charcoal. The moisture collected in the charcoal absorber bank because deficient procedures did not require that the SFU heater be on during operability testing. The cause of.the SFU operations problem resulting in the 24-hour LC0 was attributed to deficient' procedures.

Corrective Actiori:

Moisture in the "A" SFU charcoal absorber bank was removed by operating the "A" SFU with the heater on. The "A" SFU was functionally tested satisfactorily and testing indicated an acceptable filtration efficiency of 99.75%. To preclude recurrence of the moisture collection problem, changes will be made in the monthly operability demonstration procedure to require that the SFU heater be on during SFU operation. To prevent recurrence of the operations problem resulting in the 24-hour' LCO, the appropriate procedures will be modified to preclude running a declared-inoperable SFU train before first manually starting and running the redundant train.
