Event

Description:

During normal operation, the high pressure coolant injection (HPCI) system was-declared inoperable when a preliminary design review did not confirm that a loose mounting plate on spring can hanger EBB-14-H8 would not adversely affect the operability of the HPCI system. The HPCI steam supply valves were closed and the system depressurized. A 7-day limiting condition for operation (LCO) was entered as required by Technical Specification 3.5.D.2 for an inoperable

- HPCI. The hanger is located on the HPCI steam supply line. The design review was being conducted as a result of the hanger mounting plate problem having previously been identified in a nonconformance report.

Further review of the design of the HPCI system revealed that failure of spring can hanger EBB-14-H8 would not have adversely affected the operability of the HPCI. This design review concluded that in the event of the ultimate failure of hanger EBB-14-H8, the HPCI system has sufficient capacity to absorb the additional load without excess stress or unacceptable deflections. As a result of this conclusion, HPCI was declared operable again after approximately 1.4 days, concurrent with the installation of a temporary hanger near EBB-14-H8.

There have been no previous similar occurrences.

Cause

Description:

- The loose mounting plate on EBB-14-H8 was caused by the degraded condition of the " Red Head" concrete expansion bolts.

l' Corrective Action:

i A temporary hanger was installed on the HPCI steam supply line near EBB-14-H8 using "Hilti" concrete expansion bolts. After this installation, HPCI was functionally tested satisfactorily and declared operable. EBB-14-H8 will be ,

repaired and the temporary hanger then removed.

A design review will be initiated to determine if the problem with mounting '

plate EBB-14-H8 has generic implications.
