DUANE ARN0LD ENERGY CENTER Iowa Electric Light and Power Company LICENSEE EVENT REPORT-Supplemental Data Docket No. 050-0331 Licensee Event Update Report Date: 4-23-82 Reportable Occurrence No: 79-021 Event Description During surveillance testing the minimum flow valve (MOV 2009) for RHR (LPCI) pumps IP-229A and IP-229C would not open. The pumps were declared inoperable and required surveillance testing begun. A seven day LC0 was entered as per Tech. Spec. Section 3.5.A.5. Subsequent testing on September 21, 1979 demonstrated both pumps could meet head and flow requirements with M0V 2009 in the open position. MOV 2009 was, therefore, secured in the open position and the pumps declared operable, ending the seven day LCO. There have been no previous similar occurrences.

Cause Description Disassembly of Limitorque SMB-000 valve operator revealed the operator worm gear was stripped. Cause unknown.

Corrective Action The worm and worm gear were forwarded to Limitorque Corp. for examination of wear.

Their evaluation determined that wear was not caused by improper design. The worm, worm gear, quad rings and 0 rings were replaced to return the operator to operation.

A slight misalignment of the gears noticed during operator disassembly, not believed to be responsible for worm gear wear, was corrected. A higher quality lubricant was used during reassembly of the operator. The operator was inspected during the 1981 refueling outage and no problems were found. In addition a sample of similar valves were inspected during the 1981 refueling outage with no similar wear problems found.

No additional corrective action is planned.
