@ent Des _crintion Dur-ing narmal power operation, the f!RC Resident Inspector found the instrument line drain velve, V-18-20, for "C" scraa instrument volum level switch LS-1861D locked open. The valve is supposed to be locked closed. A pipe cap imnediately ~ -

- downstream of the valve was, properly installed. Followin'g a reactor s~ cram, the scram instro.7nt volume and associated piping effectively becor.as en extension of the primary s;/s ten. This event resulted in a reduction from double isolation to single isolatic. of the primary system boundary. This event alone was not  ;

sufficient to result in a loss of primary system integrity. Technical Specification 3.6.G lists the primary systern integrity requirerents. There have been no preyious occurrences involving mispositioned valves in this system.

_Cause Desc'ription

• Personnel error. The valve had apparently beca locked'open 'during perfornance of a monthly surveillance test on ll/11/81 and not locked closed as required upon

. com;iletion of the test. The testing was being done to comply with URC Bulletin 80-14.

l Upon discovery V-18-20 was locked closed. The pcrson who perfonned the test has hcen forically disciplined. '!-18-20 is a Dragon model 67055G3B gate valve.

No further corrcctive action is planned.
