Technical Specification 6.11 requires, in part, that procedures for radiation protection be prepared and adhered to for all plant operations. Licensee Procedure RPP-10 requires, in part, that survey instruments and self-reading dosimeters (SRDs) issued from the Radiological and Environmental Services (RES) Issue Room be logged out.

Contrary to the above, on February 28,1994, an SRD issued to the NRC inspector was not logged out of the RES Issue Room, and on March 4,1994, a survey meter found in the Interim Radwaste Facility was not logged out of the RES Issue Room.

This is a Severity Level IV violation.

Admission or Denial of the Alleced Violation  !

The Power Authority agrees with this violation.

The Reason for the Violation 12ck of attention to detail by RES Issue Room personnel and lack of RES supervisory oversight.

Corrective Actions That Have Been Taken:

The short term corrective actions were as follows:

The SRD was properly logged out at the issue room.

1)

2) The Ludlum Model 177 (serial no. 353) was returned to the issue room.
3) The instrument issue room technician was counseled on the importance of following approved procedures.
4) RES supervision provides periodic oversight of instrument issue to verify adherence to the procedure.

5) Radiation Protection supervisors and technicians have since attended the newly l developed Enhanced Work Practices Training which emphasizes attention to  !

detail, procedure adherence, and worker's responsibility to change procedures l that cannot be followed as written. i

6) A review of this violation and procedural adherence issues has taken place in Radiation Protection department meetings.

4 Results Achieved-l 1

1) The technicians assigned to the instrument issue room understand the importance of instrument issue control including logging instruments in/out and following up on instruments not returned on time.
2) Radiation protection supervision understand the necessity for periodically reviewing work practices associated with procedure adherence.

Corrective Steps That Will Be Taken to Avoid Further Violations:

1) RES supervisors are required to periodically perform reviews of work practices in areas associated with radiation protection procedure adherence.
2) RES supervisors are required to immediately initiate corrective actions upon 4 discovery of poor work practices and document them by initiating Deviation Event Reports (DERs) as required.

Date When Full Compliance Was Achieved:

Full compliance was achieved on March 17,1994 as the result of completing corrective actions stated above.
