Medical Restriction i
Dear Mr. Miller:
1 During a recent review of a medical record, the site medical personnel identified that Mr. Kerry S.
Allen, SOP-10089-3, did not meet the ANSI /ANS 3.4-1983 requirements for vision without corrective lenses.
This condition should have been identified in August
- 1997.
Mr.
Allen currently has no restriction on his operating license, however for his comfort he does have and use eyeglasses for reading.
Attached is an NRC-396, in triplicate, requesting that a restriction for corrective lenses be placed on his current license.
Copies of the medical information to support this restriction are included.
The medical information provided is considered personal and confidential medical files by NYPA.
Therefore, it is requested that this information be withheld from public disclosure in accordance with 10CFR2.790(a) (6).
As a result of this discovery our medical personnel have reviewed the medical records for other licensed opeators to ensure they are meeting the standards.
