At 1638 on October 2, 1989 a Unit 1 Safety Related bus was removed f*om service. The Unit I and Unit Common T;chnical Specification action statements were entered. The review did not consider the impact on Unit 2.

l- The bus was part of the AC power Sources requirement for Unit 2. The action statement provided for the bus to be inoperable for 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. This provision specified that operability of the remaining sources be verified by performing surveillance requirements at an increased f requency. perfomance of a Specification which consists of breaker alignment and bus voltage verification was required within 1 bour and every 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> thereafter. At 2053 a supervisor discovered that the Action Statement had not been entered for Unit 2 when the bus was removed from service. At 2110 the requirements of the action statement were completed with l' acceptable results. This exceeded the within one nour" f reque.ity t y 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> and 32 minutes. The cause of this event was a procedural deficiency. The Out-of-Service Procedure was structured in a unit specific f emat . This created a mind set that did not key the reviewer to consider the impact on the opposite unit.

Tha Specification was inwnediately performed. The Out-of-service procedure will be revised to include a check l

off block for the opposite unit. There have been n9 previous occurrences.

-B. DESCRIPTION OF EVENT:

At 1638 on October 2, 1989 Bus 141 (AP) ((A), one of the redundant $afety Related electrical AKV busses on Unit 1, >

cas removed f rom Service. The appitcable Unit 1 and Unit Common Technical Specification action statements were ,

entered and compiled with. The initial review for Technical Specification oppilcability did not consider the p

impact on Technical $pecifications for Unit 2.- f,,

t 4KV Bus 141 was part of the AC Electrical Power Sources required to be operable for Unit 2. This was in L

.accordance with technical $pecification 3.8.1.1. The action statement provided for Bus 141 to be inoperable for 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. This provision specified that operability of the remaining AC Sources be verified by performing survelliance requirements on the remaining AC Sources at an increased frequency. At 2053 the Station Control Room Engineer ($CRE) ($R0 Licensed Supervisor) discovered that the Action Statement for Unit 2 Technical $pecification 3.8.1.1 had not been entered when Bus 141 was removed f rom service. The SCRC immediately entered the Action $tatement.

At 2110 $pecification 4.8.1.1.la was completed with acceptable results. This exceeded the "within one hou*"

frequency by 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> end 32 minutes. All other actions were completed within the time intervals specified in the l Action Statement and found acceptable.

'This event is being reported pursuant to 10CFR50.73(a)(2)(1) - any operation or condition prohibited by the

9. SAFETY ANALY$l$:

This event had no effect on the safety of the plant or the pubilc. Specification 4.8.1.1.la is an " instant in time" verification of items which have continual monitoring and alarm function in the main control room.

Under the worst case condition of operation without the AC power Sources provided by the opposite unit's 4KV E$F busses there would sill) be no effect as this is enveloped in Section 8 of the Updated Final Safety Analysis

~R: port (UFSAR). The Diesel Generators (DG) (EK) are the Emergency Power Sources for the Unit 4KV E$F Busses. One of the two redundant DG's are sufficient for the assumptions of the UFSAR. Both Unit 2 DG's were operable and evallable for this event.

E. CORRECTIVE ACTIONS:

Specification 4.8.1.1.la was immediately performed and found satisf actory for the remaining AC Sources.

The Out-of-Service procedure will be revised to include a check off block on the Outage Forms for consideration of h Technical Specification appitcability for the opposite unit. This wlil be tracked to completion by action item 4 457-200-89-08201.
