As part of the action statement, a grab sample was required every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. Radiation Protection Department (RP) had initiated a f requency of 1 grab sample at 0400, 1200, and 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> on a daily basis. At 0400 on August 27, 1990 the ABV$E grab sample was taken in accordance with the schedule. At 0700 durleg the RP $hlf t $vpervisor (RP$$) revlem of the Duty Assignment $5.et, the RP$$ lost cognizance of the ABV$E grab sample requirement and f ailed to enter it on the sheet. At 1430 the day shif t RPs$ was relieved by the af ternoon shi't RF$$. The need to perform the 2000 ABV$E grab sample was discussed during turnover. The day shif t RP$$ assumed that the 1200 sample had been taken. At 1940 the 2000 ADVSE grab sample was taken. At 2100 the afternoon shift RP$$ identified that there was no record of the 1200 ABVSE grab sample being submitted for analysis. At 2245 af ter discussions with the day shif t RP$$ and RP Technician, it was concluded that a 1200 ABVSE grab sample had not been taken. The causes of the event were personnel error and deficient work practices. Training has been provided, programs will be modified, and the station is reviewing the non routine surveillance process. Previous corrective actions are not applicable.

There were no systems or components inoperable et the leginning of the event which contributed to the severity of the event. The Unit 1 Auxillery Building Vent Stack Ef fluent (ABVSE) Radiation Monitor (PR) (IL). IPR 02sJ, had been declared Inoperable on August 9.1990. The approp late Technical Specification Action Statements had been extered. As part of the action statement, an ABV$E gra b sample was required to be taken every 12 hours. To facilitate this requirement.. Radiation Protection Dep.rtment (RP) had initiated an adelnistrative frequency of I grab sample every 8 hours. The samples had been taken at 0400, 1200, and 2000 hours on a daily basis while the monitor was ino'perable.
At 0330 On August 27. 1990 the RP $hif t Supervisor (RP$$) (Non-Licensed RP Supervisor) directed an extra RP Tcchnician (RPT) to perfore tasks on the $hlft 2 (day shift) Duty Assignment Sheet.
At 0400 the ABVSE grab sample was taken in accordance with the administrative schedule.
At 0630 the night shift RP$$ was reeleved by the day shift RP$$. The need to perfota the 1200 ABVSE grab sample was discussed during turnover. Additionally, the Technical Specification Action Statement tracking board located in the RP Of fice reflected the nett for the 1200 sample.
At 0700 the extra RPT, who had been p rforming portions of the Shift 2 Duty Assignment sheet, submitted the sheet to the day shif t RP$$. During the RP$$ review of the Duty Assignment Sheet, the RP$$ lost cognisance of the ABVSE grab sample requirement. As a result the sample requirement was not entered on the assignment sheet.
At 1430 the day shift RP$$ was relleved by the afternoon shift RP$$. The need to perform the 2000 ABVSE grab sample was discussed during turnover. The day shif t RPS$ assumed that the 1200 sample had been taken and did not discuss it with the afternoon shift RPS$.
At 1530 the af ternoon shif t RPS$ completed the Shif t 3 (af ternoon shif t) Duty Assignment sheet. The RP$$ entered the requirement for a 2000 ABVSE grab sample on the sheet. Additionally, the RP$$ completed a Health Physics Technical Specification Tracking sheet, directing the af ternoon shif t RPi to take the 2000 sample and issued both to the af ternoon shif t RPT.
At 1615 the RP$$ reviewed the $hift 2 Duty Assignment sheet and observed that there was no entry regarding the ABVSE grab sample. Because the day shif t RPS$ had been cognizant of the sample requirement, the af ternoon shif t RP$$ assumed the sample had been taken and did not initiate any follow-up on the discrepancy at that time.
At 1940 the 2000 ABvSE grab sample was taken by the af ternoon shif t RPt.

At 221$ the discrepancy with the 1200 ABV$t grab sample was discussed at turnover. The afternoon shift RP$$ ,
co2tacted the day shift RP$$ at home and the night shift RP$$ contacted the day shift RP1 at home in an attempt to l FCsolve the discrepancy. Additionally. the afternoon shift RPT was dispatched to the Chemistry Counting Room to j look for the missing sample. j 1
At 2245 after discussions with the day shift RP$$ and RPT It was concluded that a 1200 ABy$t grab sample had not I been taken. This resulted in the action statement time interval. being exceeded by 3 hours and 40 minutes. l j
Thlg event is being reported pursuant to 10CFR50.73(a)(2)(1)(B) - any operation of condition prohibited by the plant's Technical $pecifications. '
C. CAU$C Or EVENT:
The root cause of this event was a cognitive personnel error. The RPS$ failed to both enter the 1200 ABV$t grab sample requirement on the day shift I'vty assignment $htet and initiate the Health Physics Technical $pecification ,
i scking sheet for the sample. The sample requirement was discussed prior to taking the shif t but the RP$$ lost jisance of it when reviewing the $hift 2 Outy Assignment Sheet. Then when the requirement was discussed at the A ' noon shift turnover, the RP$$ assuw d that it had been completed without either discussing it with the RPi or '
cerifying that the documentation for performance had been completed.
CGntributing cause to the event was deficient work practices on the part of the RPT responsible for takles day shif t samples. The RPT did not check the Technical $pecification Action Statement Tracking board located in the RP of fice where the sample requirement was identified. Additionally the RPT did not question the RP$$ about the status of the sample that had been assigned to him for the previous five days.
D. $AftTY ANALY$l$t This event had no ef fect on the safety of the plant or the public. Both the 0400 sample and the 2000 sample results Indicated that the activity levels from t;ie ABV$t were nonnel and within accepted levels. The action statement for the 1PR026J also requires continuous monitoring for lodine and particulate which was in operation throughout the event.
Under the worst case conditions of an accident in progress where a release through the ABVSE was occurring, the Wide Range Noble Gas Monitor, 1PR030J. continually monitors this vent path providing accident assessment and response personnel with detailed information to adequately evaluate release conditions. Both the 1PR030J and its Unit 2 counterpart the 2PR030J were operable aad available throughout this event.
