Occurrence Date April 1, 1976 Identification of Occurrence Failure of containment spray system Il torus spray valve V-21-15 to function when called upon to do so. This event is considered to be a 30-day reportchie occurrence as defined in the Technical Specifications, paragraph 6.9.2.b.2. Conditions Prior to Occurrence The plant was at steady state powcr with major plant parameters as follows: Power: Core, 1654 MWt Elcetric, 570 !#e Flow: Recirculation, 54.9 x 106 lb/hr Feedwater, 6.11 x 10 6 lb/hr Stack Gas: 6750 pci/sec  ; Description of Occurrence on April 1,1976, while performing the monthly pump operability test for containment spray system II, which was done in conjunction with the quarterly automatic actuation test, it was observed that valve V-21-15 failed to open when a containment spray auto start signal was applied. Normally, the valve opens to admit a recycle flow of cooled water from the discharge of the containment spray system back into the torus via a spray header in the torus vapor space. An operator was dispatched to the notor control center for V-21-15 nnd the circuit breaker overload reset was operated, whereupon' the valve was observed to open. -

Repo'rtable Occurrence No.- 50-219/76-10-3L . . April 26, 1976 Apparent Cause of Occurrence -

  • The cause of the overload trip was traced to a set of misaligned starter contacts on the _460-volt breaker which resulted in intermittently disconnecting one of the three phases feeding electfical power to the valve motor. .This caused a trip of the overloads which immobilized the . valve. It is not known how the contacts became misaligned, but it is suspected that it may have occurred during the annual breaker c1 caning and inspection.

Analysis of Occurrence The failure of valve V-21-15 to_ function is considered to be of minimal significance since system I torus spray valve -V-21-18 functioned satisfactorily. The torus spray which is approximately 150 gpm for each loop is intended to cool the gases which are contained.in the torus vapor space under loss of coolant accident conditions. Failure of this valve to operate resulted in a loss of 1,ystem redundancy. Corrective Action The misaligned contacts were repositioned and inspected. The preventative maintenance program that is being developed will include post-maintenance testing requirements that will insure equipraent operability following planned maintenance activities.
