This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.b(2).

Conditions Prior to Occurrence The plant was operating at steady state power. The major parameters at the time of occurrence were:

Power: Reactor, 1830 Mwt Generator, 634 Mwe Flow: Recirculation, 14.3 x 104 gpm Feedwater, 6.85 x 106 lb/hr Stack Gas: 3.19 x 104 pCi/sec Description of Occurrence Core spray booster pump N703B did not start when it should have dering a regular monthly surveillance test. The pump had properly started earlier in this same test. The technician checked the breaker control power fuse, which had been removed and reinserted as part of the test. After checking the fuse, wh*ch was apparently loose in its holder, the pump started. A job order was initiated to change the fuse and check the tightness of the fuse clips.

The fuse was replaced and the automatic actuation of the pump was reverified to confirm proper operability.

Apparent Cause of Occurrence 1511 30B Unable to determine whether personnel error or equipment defect wees the cause of the event.

.; portable Occurrence No. 50-219/79-37/3L-0 Page 2 November 30, 1979 Analysis of Occurrence The Core Spray System is an emergency system designed to provide continued cooling of the reactor fuel during loss of coolant accidents. There are two redandant loops, each capable of performing the design function. Each loop has two redundant sets of pumps, each capable of performing the design function. One set in each loop will automatically start in response to emergency signals. The other set in each loop is in reserve and will automatically start if the primary set does not. The safety significance of this event is minimal because the ability to deliver full design flow to the reactor was continuously provided by the core spray loop not being tested.

Corrective Action A Job order was initiated to check the rest of the fuses in the plant which are manipulated during surveillance tests and could affect operability of safety systems. No abnormalities were found. The instrument and control technicians will be informed of this problem and reminded to check the tightness of fuses when removing or reinstalling them.
