This event is considered to be a reportable occurrence as defined in tha Technical Specifications, paragraph 6.9.2.b.2.

Conditions Prior to Occurrence The plant was in a refueling shutdown.

Mode switch positicn - refuel.

Reactor coolant temperature - 171.8 F.

Description of Occurrence On December 4, 1978, at approximately 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br />, during Containment Spray System I automatic actuation test, valve V-21-18 failed to open. Normally, the valve opens to allow about five percent of recycled coolant flow discharge through a spray header in the atmosphere over the absorption pool.

Apparent Cause of Occurrence The valve motor breaker was not racked in properly and the overload was found in the trip position.

Analysis of Occurrence The failure of valve V-21-18 to function is considered to be of minimal significance, since the containment spray cooling system consists of two, independent, cooling loops, each capable of removing fission product decay Jersei Centra: Power & Lign: Company is a M "N- O' tne Gene a! Reportable Occurrence No. 50-219/78-32/3L-0 Page 2 Janua ry 3,1979 heat f rom the prima ry contai nment. Eaca loop consists of a pair of spray headers in the drywell vessel. Each pair of spray headers is fed by two pumps and two heat exchangers. Either pump and either heat exchanger in a loop is capable of removing fission product decay heat from the primary containment system. Failure of this valve to operate resulted in a loss of system redundancy.

Corrective Action The valve motor breaker was racked in properly and the notor overload was reset. The valve was then tested for operability and the amperage readings were found to be normal. Additionally, a procedure is being generated giving detailed instructions concerning racking breakers in and out.
