CAUSE DESCRIPTION 4 Time delay trip relays were out of adjustment and output breakers, GE

>] '"~hi type TFK2267225 with UV relhlse, dz.d not operate correctly. The time delay relay was recalibrated and the output breaker reset to activate the UV trip. The reset of the output breaker requires a unique sequence which was not recognized initially. Investigation of the failure to trip revealed that the reset instructions requiring the breaker trip coil to be energized when the breaker is reset were on the side of the breaker and difficult to see with the breaker in place. This operating requirement was not generally known. The proper reset sequence has since been conspicuously posted at each breaker. (BFRO-50-296/775)
