On March 18, 1994, Unit 3 was in Mode 1 (POWER OPERATION) at approximately 86 percent power with one Main Steam Safety Valve (MSSV) gagged when APS Engineering personnel completed Trevitesting of 13 of the 20 MSSV. The relief settings on 7 of the tested valves were found below the allowed - 1 percent tolerance limit specified in Technical Specification 3.7.1.1. All 7 valves were adjusted, certified, and returned to service. The relief setpoints of the other 6 valves were found within the allowed +/- 1 percent limit. One of these valves could not be certified and was left inoperable. On March 19, 1994, Unit 3 was shutdown for the start of the scheduled refueling outage, 3R4. During this outage the remaining 7 untested MSSVs and 4 Pressurizer Safety Valves (PSV) will be removed and sent to the Westinghouse service facility for testing. A supplement to this LER will be submitted when testing of all the Unit 3 MSSVs and PSVs has been c'ompleted and analyzed by APS Engineering.

The most recent LERs on MSSV Out-of-Tolerance conditions are 528/93-009-02, 529/91-005-01, and 530/92-005-00.
