Upon discovery, a satisfactory leak test was performed and completed at approximately 1630 MST on October 6, 1988.

The cause of this event was a cognitive personnel error on the part of an ANPP Technician in that he inadvertently identified a 97.7 millicurie source as being a 97.7 microcurie source for surveillance tracking purposes.

As corrective action, all other entries in the Source Tracking System were verified to be correct. Information entered into the Source Tracking System will be independently verified. This event will be reviewed by personnel responsible for maintaining the Source Tracking System.

There have been no previous similar events reported pursuant to 10CFR50.73.

DESCRIPTION OF WHAT OCCURRED:

A. Initial Conditions:

At the time of the event on September 20, 1986, Palo Verde Units 1 and 2 were in Mode 1 (POWER OPERATION) at approximately 100 percent power. The event occurred in Unit 3 on March 25, 1987 when the Operating License was issued.

B. Reportable Event Description (Including Dates and Approximate Times of Major Occurrences):

Event Classification: Condition Prohibited by the Plant's Technical Specifications At approximately 1600 MST on October 6, 1988, an ANPP Radiation Protection (RP) Technician (utility, non-licensed) discovered that a clerical error in ANPP's Source Tracking System resulted in a radioactive sealed source not being leak tested pursuant to Specification requirements. This leak testing was 'echnical required to have been performed by September 20, 1986. A satisfactory leak test was performed and completed at approximately 1630 MST on October 6, 1988.

Technical Specification 3.7. 10 requires that each sealed source containing radioactive material in excess of 100 microcuries of beta and/or gamma emitting material be free of greater than or equal to 0.005 microcurie of removable contamination. Associated Surveillance Requirement 4.7. 10 '(a) requires that sources in use be surveillance tested at least once per six (6) months.

On October 6, 1988, Unit 2 Radiation Protection personnel (utility, non-licensed) returned a radioactive sealed source to the ANPP Central Radiation Protection Calibration Lab. The source had been stored in Unit 2 and was periodically used by. Unit 2 personnel for Radiation Monitoring System (IL) operational checks. However, due to the infrequency of use, Unit 2 personnel decided to transfer the source to the Central RP Calibration Lab for storage. When the source was delivered to the Central RP Calibration Lab, the individual (utility, non-licensed) updating the computerized Source Tracking System noted that the source was improperly listed as a 97.7 microcurie source. The source is actually a 97.7 millicurie source. Because the source was improperly listed as being less than 100 microcuries, no surveillance testing had been performed since the source was delivered to ANPP on February 2; 1986 (appropriate leak testing was satisfactorily performed upon delivery).

Following the discovery that the source had not been properly surveillance tested, appropriate leak testing was immediately o med 'ccordance with a roved rocedural controls.

Not applicable - no structures, systems, or components were inoperable which contributed to this event. However, the sealed source leak testing was performed upon receipt on February 2, 1986 and subsequently on October 6, 1988. The .surveillance interval for the sealed source expired on September 20, 1986.

Cause of Event:

The root cause of this event was a cognitive personnel error on the part of an ANPP Technician (contractor, non-licensed) responsible for maintaining and updating the Source Tracking System. The source was delivered to ANPP on February 2, 1986, and upon receipt inspection, information concerning the source was entered into the Source Tracking System. The individual completing the documents utilized for source tracking inadvertently identified the 97.7 millicurie source as being a 97.7 microcurie source. Since it was perceived that the source was less than 100 microcuries, no tracking of the source was required for surveillance purposes.

The error was not a result of an error in an approved procedure and was not a result of an activity or task not covered by an approved procedure. There were no unusual characteristics of the work location that directly contributed to the event.

J. Safety System Response:

There were no safety system responses and none were necessary.

K. Failed Component Information:

Not applicable - there were no failed components.

II. ASSESSHENT OF THE SAFETY CONSEQUENCES AND IMPLICATIONS OF THIS EVENT:

There were no safety consequences or implications resulting from this event as there was no impact on the safe operation of the plant or the health and safety of the public. Leak Testing performed as a result of this event determined that source leakage in excess of Technical Specification limits had not occurred.

I I I. CORRECTIVE ACTIONS:

A. Immediate:

The radioactive source was appropriately leak tested satisfactorily and the Source Tracking System was updated with the correct information.

B. Action to Prevent Recurrence:

Information contained within the Source Tracking System was reviewed and verified to be accurate. Additionally, an independent verification will be performed on new information entered into the Source Tracking System to ensure that the data is accurate. The individual responsible for maintaining the computerized Source Tracking System in February 1986 is no longer employed at PVNGS.

This event will be reviewed by personnel currently maintaining the Source Tracking System.

There have been no previous similar events reported pursuant to 10CFR50.73.
