At 1543 MST on March 2, 1987, Palo Verde Unit 1 was in Mode 4 (HOT SHUTDOWN) when a hourly fire watch patrol was 'not performed in accordance with Technical Specification 3.7.12 ACTION a. The fire watch patrol was 4 minutes late in making the hourly patrol of the 80 foot level of the Main Steam Support Structure.

The root cause of this event was a cognitive personnel error by the swing shift Building Services Foreman in that he made a change in the fire watch rounds and did not ensure that the fire watch received the change in time to satisfactorily perform the patrol.

As corrective action to prevent recurrence the foreman involved was disciplined and instructed on the importance of being cogni.zant of all'fire watch activities and ensuring fire watches are first priority. All other Building Services Foremen were also notified of the importance of the fire watches.

The fire watch patrol (contractor non-licensed) was 4 minutes late in making the. hourly patrol of the 80 foot level of the Main Steam Support Structure (MSSS). Subsequent patrols were completed within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, thus complying with the Technical Specification requirement. The event lasted approximately 4 minutes.

The fire watch patrol had been established as a compensatory measure due to the potential inoperability of Thermo-lag'ire barriers. The fire barriers had been declared inoperable pending an evaluation for acceptability of the as-installed Thermo-lap.

At 1500 on March 2, 1987, the swing shift Building Services Foreman (utility non-licensed), responsible for the conduct of fire watches, had incorporated the MSSS fire patrol with another fire patrol in order to gain maximum man power benefit. This incorporation was possible due to the termination of another fire patrol that was part of the MSSS fire patrol rounds. During day shift there were two fire watch personnel performing the patrols. After incorporating the two fire watch patrols into one, there was only one fire watch individual to perform the fire watch patrols. The Foreman did not transmit the change in the patrols to the fire watch in time for the fire watch patrol to satisfactorily perform the patrol within the time constraints required by Technical Specifications.

The root cause of this event was a cognitive personnel error by the swing shift Building Services Foreman (utility non-licensed) in that he made a change in the fire watch rounds and did not ensure that the fire watch received the change in time to satisfactorily perform the patrol. Not completing the assigned rounds in the required time is contrary to approved procedural controls. The current procedural controls have'een previously evaluated and are considered adequate.

There were no errors in the procedure which contributed to the event.

As corrective action to prevent recurrence the foreman involved was disciplined and instructed on the importance of being cognizant of all fire watch activities and ensuring fire watches are first priority. All other Building 'Services Foremen were notified of the'importance of the fire watches.

There were no structures, components, or systems that were inoperable at the start of the event, other than those described above, that contributed to the event. There were no manual or automatic safety system responses. There were no unusual characteristics of the work location which contributed to the event.

There were no operator actions which contributed to this event. This event did not adversely affect the safe operation of the plant or the health and safety of the public since in the event of a fire, the required area fire detection (DET) and suppression (SPNK) systems remained operable. A similar event where the root cause was attributed to personnel error by a Building Services Foreman was reported in LER 86-057-00. In the previous event the Foreman did not assign a required fire watch patrol. Corrective action in the previous event involved the installation of status boards in the foreman'st trailer. In this current event the fire watch patrol was assigned and correctly .

statused however, the chang'e in the patrol was not give'n to the fire watch in sufficient- time to complete his round satisfactorily. Therefore, the previous corrective action would not have prevented this event.
