LICENSEE EVENT REPORT PAGE TWO ADDITIONAL CAUSE DESCRIPTION AND CORRECTIVE ACTIONS Thc S.I;T. Remote Sampling System was installed in Oct. 1979. The sample valve mislabelin'g problem went undetected until Feb. 1980 because the normal sample volume is too small to change S.I.T. level indication.

Since the S.I.T. samp1ing valves are within plant containment, the specific error was. not confirmed until the 'preserit refueling'utage.

proved that the system piping and valves were correctly 'nvestigation installed per the Hechanical Design Drawings and the system control wiring. was correctly installed in accordance -with the Electrical Design Drawings. However, the system designer had inadvertently transposed the mechanical-electrical interface (inside containment versus outside) resulting in a mirror-image mislabeling problem.

This error was an isolated design problem and the cognizant design review personnel have been made aware of the cause to prevent reoccurrance of similar errors. This is the first event involving surveillance errors due to this type of problem.
