i Reportable Occurrence 335-80-4 Licensee Event Report Pa e Two Additional Event Descri tion At about 3:00 p.m. on 1/24/80 it was noted that a feeder breaker (2B4-5) from the B startup transformer to the B 4160V vital bus was open. This condition had existed since 5:00 p.m. on 1/18/80 and was not in compliance with T.S.

3.8.1.1 Action a. For 5-1/2 hours on 1/24/80 the A D/G was taken out of service for periodic maintenance without the knowledge of the condition of the "B" startup transformer. However, the surveillance requirements and time limits of T.S. 3.8.1.1. Action b were met. Also, the B D/G was tested satisfactorily on 1/18/80 and again on 1/24/80 so it was operable during the period in question as was the A startup transformer, and (except for the period on 1/24/80) the A D/G. The Startup transformer was energized from normal offsite power. The vital bus is (and was) normally energized during operation from the main generator through the auxiliary transformer. There is only local indication that the feeder connecting the two (through an automatic breaker) was de-energized. Although the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> limit was exceeded it must be pointed out that, when the condition was noted it was corrected within about 15 minutes.

Additional Corrective Actions In addition to the corrective actions listed on LER 335-80-4, more administra-tive control over the subject breakers has been obtained by locking the startup transformer feeder breaker enclosures and directing that no switching orders be implemented on the startup transformers without the approval of the operations supervisor until refresher training has been conducted.

Breaker 284 is intended for maintenance use only and is not remotely operable nor is there any remote position indication. The breaker and its counterpart in the A electrical train are now specifically detailed in the surveillance procedure and must be visually (locally) verified closed every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.
