MEMO FOR FACSIMILE TRANSMISSION TO CONFIRM PROMPT REPORTABLE OCCURRENCE 335-79-31 This memo confirms our telephone notification of the subject 14-day reportable occurrence made on September 27, 1979 to Hr. J. Dyer of your office.

While shutdown during the recent plant outage, a QA/QC verification of installation of new Safety Injection Tank sample lines revealed that four (4) raceway clips had been welded to the containment vessel wall. The clips were approximately 1/2" X 2" in size and were employed to provide support to the 3/8" sample line. The clips were installed during the Spring, 1979 refueling outage, but were not noted to be improperly installed until completion of the modification in September, 1979.

'The clips were removed from the containment vessel wall by grinding and have been re-installed in a proper location. The wall area of the removed clips was heat-treated and non-destructively tested (PT) after heat-treating to verify wall integrity and to assure that no further action was required.
