MEMO FOR FACSIMILE TRANSMISSION TO CONFIRM PROMPT REPORTABLE OCCURRENCE 389-83-005 ISOLATED SAFETY INJECTION TANKS This memo confirms the telephone call at 9:47 P.M. on 5/7/83 with the NRC Duty Call Supervisor.
At about 9:15 P.M. on 5/7/83, it was noted that the SIT isolation valves were closed with the plant in Mode 4 above 276 psia which is not in compliance with TS 3.5.1.a.
The valves were immediately opened.
The condition had existed for 4.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />. The apparent cause is personnel error in that a procedural step had been overlooked during plant heatup for post core load hot functional testing. The plant was at about 260 F and 400 psia. It should be noted that the plant has not been critical and thus has no decay heat and that these valves open automatically at 515 psia.
