On February 18, 1988, St. Lucie, Unit I2 was in Mode 1, 100% power and at steady state conditions. During a. routine surveillance inspection by plant Quality Control personnel, the Reactor Containment Building IRCB) (EIIS-NH) polar crane power supply breaker was discovered to be in its required rendered inoperable open position, tagged in the open position with a controlled clearance tag, but was not locked in the open position. This constituted a violation of the plant's Technical Specification and plant operating procedure.

The root cause of the event was a cognitive personnel error on the part of the Assistant Nuclear Plant Supervisor who reviewed the surveillance procedure performed on the RCB polar crane power supply breaker, by misinterpreting that the clearance tag hung on the breaker had met the intent of the Technical Specifications as well as the plant operating procedure. A contributing factor to the misinterpretation is the fact that the RCB polar crane power supply breaker for St: Lucie Unit I)1 is not required by procedure nor by the Technical Specifications to be locked open, but rather only required to be tagged open by means of a controlled clearance tag.

As an immediate corrective action, the RCB polar crane power supply breaker was locked open. "

St. Lucie Unit N2 remained at mode 1, 100% power during the event. At no time was the plant safety nor the health and safety of the public endangered.

DESCRIPTION OF EVENT.

On February 18, 1988, St. Lucie Unit II2 was at mode 1, 100% power, and at steady state conditions. During a routine surveillance inspection by plant Quality Control personnel, the Reactor Containment Building (RCB) (EIIS-NH) polar crane power supply breaker I)2-40514 was found tagged in the open position with a controlled clearance tag but not locked, The plant's Technical Specifications as well as plant operating procedures require that this breaker shall be rendered inoperable by locking the power supply breaker open, while the plant is in modes 1 and 2. St. Lucie Unit I)2 had just completed its refueling outage November 21, 1987, and by procedure the RCB polar crane was to have been verified as being rendered inoperable by having its power supply breaker in the locked open position and have a controlled clearance tag to be in the open position prior to entering mode 2. Furthermore, a monthly surveillance is performed on this breaker to ensure The latest surveillance performed was on February 16, 1988.

Investigation revealed that there was no reason for removing the breaker lock from the time of its last performed surveillance to the time of discovery. It was conservatively presumed that the breaker was rendered inoperable by means of the controlled clearance tag only but not locked. It could only be speculated that the breaker was never locked since November 21, 1987.

CAUSE OP THE EVENT:

The root cause of the event was a cognitive personnel error on the part of the Assistant Nuclear Plant Supervisor who reviewed the surveillance procedure on the RCB polar crane power supply breaker. He misinterpreted that the clearance tag hung on the breaker had met the intent of the Technical Specifications as well as of the plant operating procedure. A contributing factor to the misinterpretation was the fact that the St. Lucie Unit IIl Technical Specifications as well as the'Unit I)l operating procedure do not require the Unit I)1 RCB polar crane breaker to be locked. It is only required to be in the open position with a controlled clearance tag. This 'personnel" error was a result of not adequately following an approved procedure. There were no unusual characteristics of the work location that directly contributed to this error.

ANALYSIS OP THB EVENT:

The as found condition of the breaker being in the unlocked position has no significant impact on the safety of the plant. In some cases where certain equipment is required to be in the locked position but can not be physically locked, the controlled clearance tag is considered as an acceptable alternate for the locking device and considered as meeting the intent of the locking requirement. In this event involving the RCB polar crane power supply breaker, the unlocked condition of the breaker would not have any impact since the controlled clearance tag serves the same purpose as the locking device. By plant procedure, no equipment rendered inoperable under a controlled clearance tag can be operated without releasing the clearance and approved by the Nuclear Plant Supervisor or his designee. At no time during the event was the health and safety of the public as well as the safety of the plant endangered.

CORRBCTIVB ACTIONS =

l. Operations personnel immediately locked the RCB polar crane power supply breaker.
2. The individual was advised of the proper interpretation of the Technical Specifications as well as of the procedu're.
3. The Operations Supervisor will assess the need for further training of operations personnel on this item.
