Corrections 'in-NIS 1 and NIS 2 Reports for Fort Calhoun Station
-In Reference 2. Omaha Public Power District (OPPD) transmitted the ISI results Lfor the Fort Calhoun Station 1996 Refueling Outage, including Form NIS-1 "0wner's 4 Report.for Inservice Inspections" and Form NIS-2 "0wner's Report for Repairs or Replacements." These reports contained typographical errors which do not reflect procedural or: ASME Code non-complian':e. for the associated activities. These typographical. errors were discovered during an. audit of OPPD's IST documentation.
The purpose of this letter is to provide corrected data for the report forms.
Attached are _Page 1_ of the NIS-1 report attachment' and Page 2 of the NIS-2 report attachment with the erroneous data circled. On the NIS-1 form, three 70 degree LUltrasonic (UT) Examinations on the' Reactor Vessel Head Flange were ideutified as " Axial-Circumferential" exams when they should have been identified as "Near s Surface" exams. On the NIS-2 form, the non-destructive examination associated
. with Modification MR-FC-95-018 was recorded as a Dye Penetrant (PT) exam when it ishould have been recorded as a Visual (VT) exam. Corrected report data for the
