Inspection Summary Inspection on August 21-24, 1979 (Report No. 50-546/79-15; 50-547/79-15)

Areas Inspected: Surveillance of the site to verify that activities in progress complied with the order confirming suspension of construction of safety related areas dated August 15, 1979; and concrete patch removal and testing activities. This inspection involved a total of 39 inspector-hours on site by one inspector.

Results: No items cf noncompliance or deviations were identified.

Funstional or Program Areas Inspected 1.

Construction Activities Surveillance of construction activities at the site indicated that all Category 1 (i.e., safety related work) had been terminated except for the protection of installed or in place components and the testing activities performed on the in place concrete and patches.

Work is being performed in non-safety related areas of the plant.

Concrete Patch Removal The licensee had selected 19 suspect and what they considered 38 good patches to be tested by the pulse echo test. This was done ta verify the accuracy of the test method. The RIII inspector was present during the testing of five patches on August 22, 1979, and the testing of one additional patch on August 23, 1979. Each patch was first checked using the pulse echo method, a field sketch was made of the patch and Mr. Muenow predicted what would be contained in the patch by documenting these predictions on the sketch.

Aft.'r that, the test results were verified by chipping the patch out.

Mr. Muenow predicted that five of the six patches would have defects in them and that the sixth patch appeared to be bonded and solid.

Radiographic Film Washing The licensee approached the RIII inspector with a request that they be allowed to re-radiograph piping that was constructed by Cherne Contracting Corporation since the quality of the film was questionable due to the method used for final washing. The request to perform this radiography was denied. The concerns in regard to this problem are documented on PSI Field Corrective Action Report FCAR No. 738, dated February 28, 1979, and on Cherne Nonconformance Report NCR 6.77 dated June 5, 1979. An additional problem is also identified

,

with Chicago Bridge and Iron and is documented under Field Nonconformance Report FNR No. 115, February 21, 1979. This item is considered unresolved.

Exit Meeting The inspector met with site staff representatives (denoted in the Persons Contacted paragraph) at the conclusion of the inspection on August 24, 1979. The inspector summarized the scope and findings of the inspection.
