NRC INSPECTION REPORT 50"529/93-14 On March 17-25, 1993, an NRC Region V Augmented Inspection Team (AIT) conducted an inspection at the Palo Verde Nuclear Generating Station, Unit 2, of your'esponse to the March 14, 1993 steam generator tube rupture event.

The AIT examined activities authorized by NRC License NPF-51.

At the conclusion of the inspection, on March 25, the inspectors held a discussion of their findings with you and members of your staff identified in the enclosed report.

Areas examined during this inspection are identified in the enclosed report.

Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspectors.

Overall, the AIT concluded that youx response to the steam generator tube rupture (SGTR)

succeeded in'bringing the unit safely to a cold shutdown condition and limiting the release of radioactivity such that no threat to public health and safety occurred.

However, our review identified several areas of concern regarding your staff's response to the event.

The most significant are in the areas of:

(1) Emergency Operating Procedure adequacy with respect to the operator's ability to use the'procedures to reliably and consistently diagnose a

SGTR event; (2) your 'conformance with the provisions of your event classification procedures

.and radiation monitoring system alarm response procedures; (3) your ability to conduct timely and accurate personnel assembly and accountability in accordance with your procedures; and (4) the effectiveness of your training programs to train your operators in all aspects of actual plant

'response which may differ from the training received in the simulator.

Any enforcement action resulting from this inspection will be the topic of separate correspondence.
