M. Allen, Unit I Plant Manager D. Canady, Manager, ANPP Communications Mana ement Discussion A management meeting was held on May 2, 1988, at the Palo Verde Nuclear Generating Station.
A summary of each area discussed is provided below.
The meeting agenda is provided as enclosure (2).
Prior to the meeting, the NRC participants toured the site extensively to personally assess the material condition of the units and to witness conduct of operations.
Licensee Event Re orts The ANPP Licensing Director presented a graphical summary of the Licensee Event Reports (LERs)
and the responsible managers presented an explanation of the individual LERs that had been submitted or were planned to be submitted since the last ANPP/NRC management meeting on February 29, 1'988.
A quarterly trend of LERs submitted indicated a recent r ise in the total number of LERs.
It'was also noted that several of the recent LERs addressed
, significant events or issues.
The licensee stated that a detailed review was being initiated to specifically identify why the number of LERs with a root cause in the support groups were so high.
Mr. Taylor and Mr. Martin expressed concern over the poor trend of LERs.
Mr. Taylor cautioned the licensee to also examine other internal indicators of performance problems, which did not reach the threshold of the LER reporting requirement.
U dates on Previousl Discussed Issues The latest status of several long term issues was provided by the licensee.
These issues included the reduction of inappropriately lighted annunciators, management control of the backlog of maintenance items, actions being taken to improve the effectiveness of the system engineer program, the progress of the technology transfer program, and actions being taken to improve the training of licensed operators.
The licensee reported that satisfactory progress was being made in each area.
As in a previous meeting, Mr. Martin again emphasized the importance of ANPP having a strong engineering organization to support their relatively complex units.
Mr. Taylor questioned whether the licensee had identified any further engineering errors following the engineering problems which led to the steam driven auxiliary feedwater (AFW) pump in both Units I and 2 being inadvertently rendered inoperable during late 1987.
Mr. Butter replied that no other significant errors had been identified and provided his opinion that the AFW problem had been an isolated incident.
Mr. Martin and Mr. Taylor both responded that they were very skeptical that no other problems existed.
Mr. Martin made clear to the licensee that improved performance could not be obtained if ANPP managers were not willing to view problems in a self critical manner and encouraged the licensee to learn from their mistakes.

Recent Events The licensee discussed the major events that had occurred since the last management meeting on February 29, 1988.
The Unit I Plant Manager discussed
'he April 19, 1988 reactor trip which occurred after equipment failure and personnel error resulted in an inadvertent opening of the motor operated disconnect for the main generator, while the unit was at 1005 power.
The Unit 2 Plant Manager discussed the bent control element assembly extension shaft, which occurred on April 17, 1988 while installing the upper guide structure into the Unit 2 reactor vessel.
The problems with the subsequent removal and replacement of the extension shaft were also addressed.
The Auxiliary Feedwater pump impeller problem, that was identified on the Unit
motor driven AFW pump, was discussed by the Director of Standards and Technical Support.

Inte rated Licensee Actions Mr. Haynes discussed actions being taken or planned to improve the overall performance of the Palo Verde site.
These actions are placed in three major categories, the first of which is to improve the way personnel feel about their jobs.
Actions to be completed in this area included:
Assess the significance of all work backlogs and take action to reduce the backlogs, as appropriate.
Improve modification retest criteri Upgrade the knowledge level of work planners.
Provide a higher level of review of computer generated work orders.
Reconsider the philosophy of procedure writing.
Reconsider the qualifications of personnel in critical jobs.
Embrace the INPO initiative to increase professionalism.
The second, major category is management oversight.
Actions planned include:
Review the workload of various groups.'eview various committee memberships.
Re-review selected past incidents from a human factors standpoint.
Emphasize the need for professionalism.
Limit procedure revisions to the extent practical.
Review/integrate the various oversight groups.
The third major area includes actions to improve the Palo Verde work environment, as follows:
Finish site construction by removing debris, providing more lighting, etc.
Appoint a site director to control and integrate activities which affect personnel.
Continue to assess the effectiveness of the reorganization.

~Sumna r Mr. Martin stated that ANPP's performance had not improved at the rate previously anticipated and that many indicators tended to predict a
future decline in overall performance.
Mr.. Martin observed that to continue to improve, ANPP needed to ensure that the senior managers could react to unexpected problems, while remaining aware of and personally involved in the routine site activities.
Mr. Martin emphasized the need for strong participation from both the oversight organizations and the engineering groups.
He suggested that ANPP reassess their recent problems using both internal resources and an external organization'uch as INPO.
Mr. Taylor warned the licensee that other licensees across the country who had allowed their performance to decline significantly had eventually incurred government mandated improvement programs and, in several c'ases, prolonged plant shutdown ANPP/NRC MEETING
