OF MEETING WITH ANPP MANAGEMENT

This refers to

a meeting held with Mr. M. DeMichele, yourself, Mr. J. G..Haynes,

and other members of your staff, with Mr. J. Taylor of the

NRC Executive Director

for Operations Office, myself,

and other members of the

NRC staff, at the

Palo Verde Nuclear Generating Station,

on May 2, 1988.

The subjects

discussed

are

summarized

in Meeting Report Nos. 50-528/88-21,

50-529/88-21,

and 50-530/88-20,

enclosed

herewith.

As we discussed

with you during this meeting,

we are deeply concerned

by a

number of recent events

which reflect poorly on the performance of Palo Verde.

Since this meeting,

several

more events

have occurred which have reenforced

our

concerns.

These events

include the early reactor criticality at Unit 1; the

discovery of the inoperable condition of both trains of the Essential

Chilled

Water System at Unit 1; the improper valve alignment for the chemical addition

tanks for Unit 1, Essential

Chilled Water System,

and Unit 3, Essential

Cooling

Water System,

and the apparent radiation overexposure

of a worker at Unit 2.

We understand

that you have

a number of actions

planned or in progress,

as

summarized

in this report, to improve your performance.

We intend to closely

monitor your progress

in this regard.

We strongly encourage

you to ensure that

your management

team is fully committed to and capable of taking the actions

needed

to reestablish

a steady trend of improving performance.

In accordance

with 10 CFR 2.790(a),

a copy of this letter and the enclosure

will be placed in the

NRC Public Document

Room.

Should you have any questions

concerning

our minutes of the meeting,

documented

in the enclosed

inspection report,
