1. Statement of Violation
!
10 CFR Part 50, Appendix B, Criterion II, Quality Assurance Program, requires that, " Controlled conditions include the use of appropriate equipment; suitable environmental conditions for accomplishing the activity, such as adequate cleanness.
." The licensee is committed'

to Regulatory Guide _1.39, and ANSI 45.2.3.
These documents require that activities be performed under suitable conditions of cleanliness.
f Construction Site Procedure (CSP) 12, " General Instruction for-Housekeeping During Construction," Revision 6, dated November 19, 1987, paragraph 8.05.03.01 states, "The work area shall be kept sufficiently clean and orderly that construction activity can proceed *
an efficient manner that will produce and maintain quality in conforma.: ; with specified requirements."
Contrary to the above, during an inspection of Unit 2 Isolation Valve i
Cubicles (IVC) on April 20 and 21, 1988, the "B" IVC was found to be below the minimum acceptable level for cleanliness and general
.
housekeeping cetivities.
Specifically, excessive amounts of construction debris, abandoned tools, and supplies were scattered throughout the "B" IVC.
Excessive quantities of dirt and dust were observed in the cable trays, under the grates, and on the beams and flanges throughout the general area.
II. Reason for the Violation The debris identified consisted of concrete dust, chips and insulation materials resulting from the high level of construction activity present in this area. The root cause for this deficiency was the crafts' failure to adequately protect cable trays and maintain area cleanliness during activities that generate debris, i
III. Corrective Action Taken and Results Achieved Immediately upon notification of the discrepant conditions in the IVC, cleanup was initiated.
Building Superintendents have been directed to pay strict attention to cable tray, components, equipment and area cleanliness, and to take necessary action as required.
Cleanup has been completed in the "B" IVC.
IV. Corrective Steps Taken to Prevent Recurrence i
1.
Building Superintendents have been reinstructed to the requirements of CSP-12, Paragraph 8.05.03: Cleanliness.

- Cement Finishers, Laborers and Cc:ntractors involved in activities that generate concrete dust and debris have been verbally instructed regarding the importance and requirements to protect cable trays and equipment during these operations and to maintain clean work areas.

Personnel have been allocated to the building Superintendents to perform required cleanup and maintenance of cable trays during daily:
cleanup activities.
V. Date of Full Campitanco STP is in full compliance at this time.
