Thank you for your letter of April 8,1988, in response to our letter and the attached Notice of Violation Dated March 9, 1988.
-We have reviewed your replies to Violations 498/8809-04 and 498/8809-05 and found them to'be responsive to the concerns raised; however..the reply to Violation 498/8809-04 did not provide sample sizes for the reviews and verifications performed in corrective action Items B, C, and D.
This was discussed by telephone with Mr. S. M. Head of your Licensing Department on May 23, 1988, and he indicated the numbers would be available during any followup inspection.
We will review the implementation of your corrective actions during a future inspection to determine whether full compliance has been achieved and will be maintained.
We do not agree with your exception to Violation 498/8809-06.
This also was discussed with Mr. S. M. Head and other members of your staff on May 23, 1988. During this discussion you indicated that you did not recognize that failure to follow the note in Step 4.7 of the test procedure was a procedural noncompliance.
Initial conditions required for the Auxiliary Feedwater Proof Test were reestablished in order to feed the steam generators as permitted by the note in Step 4.7 of the test procedure.
Upon resumption of the test, the Test Coordinator failed to reestablished the initial conditions required by the same note, in that the testing of the path in question was started without first manually closing the Flow Control Velve (FCV) against its mechanical stop. Based on our May 23, 1988, discussion, we understand you have withdrawn your exception and will issue a revised response.
