Areas Inspected: Routine announced inspection of an employee's concern of Clinton Station foundation subgrade preparation and concerns of certain employees not having access to NRC. The inspection involved a total of 22 inspector-hours by two NRC inspectors.

Results: Of the two areas inspected, no items of noncompliance or deviations were identified.

Review of Personnel Concerns With Regard To Contacting NRC On site craft personnel contacted the NRC RIII resident inspector with

concerns that they were not being allowed to contact the NRC to discuss matters that they may have concerning safe construction practices.

During the entrance meeting, the inspectors met with the licensee and i

his subcontractors to emphasize the need to post the NRC regulations, as required by 10 CFR Part 21, paragraph 21.6, in conspicuous areas throughout the site for all personnel to read.

In addition, the

inspectors requested that management and supervision advise their i

subordinates that they are not restricted from contacting NRC repre-sentatives in matters concerning safe construction practice.

Prior to departing the site on July 10, 1980, the licensee committed to contact his personnel and all on-site subcontractors to emphasize the

'

posting of the required regulations and to assure that no restrictions, orally or in writing, be placed with regard to personnel contacting the NRC.

Review of Personnel Concern Regarding Foundation Subgrade On-site craft personnel contacted the NRC RIII resident inspector with a concern regarding the Clinton Station foundation subgrade preparation, which commenced in early to late 1976.

In an effort to investigate the concern, RIII inspectors made contact with the individual and researched the quality control records to ascertain that all conditions during the subgrade preparation were identified, evaluated, tested and accepted before subsequent work was performed.

e The conditions described by the individual were such that in 1976 a

truck subsided into loose material. The concern was whether this condition was properly dispositioned and corrected in a manner not to affect the safe operation of the foundation of the plant.

An investigation into the above areas was performed on-site-during July 9-10, 1980. The following specific areas were reviewed:

Final Safety Analysis Report (FSAR) Section 2.5.4.5.1.4, " Excavation Base Treatment", discusses that the subgrade was considered adequate when the in place density exceeded a minimum of 85% relative density at location consisting of sand and 130 PCF for cohesive material.

It also states, " Additional excavation was performed where subgrade condition could not be improved. Some localized pockets of sand that could not be compacted to meet the requirements were removed and dental work was performed using a flyash backfill mixture. Figure 2.5-375 illustrates the areas that received the flyash backfill."

In addition, FSAR Attachment B2.5, " Sand Bases Under Category I Structures",

discusses sand base conditions and subsurface condition that required excavation and corrective measures to provide suitable subgrade preparation for the site.

The above licensee FSAR submittal will be reviewed in detail during the licensing review performed by NRR office of the NRC.

The quality control records for foundation subgrade elevation were

reviewed. Subgrade preparation records, drawing S21-1405, Revision A, and Bash (flyash, cement, sand, and water mixture) and excavation

-

records were reviewed and found acceptable.

Conditions that were determined not to meet the subgrade requirements by the on-site soils engineer were properly identified, recorded, evaluated and repaired according to Specification K-2942, Section 306.10. This section requires Bash material to be placed where excavations were carried deeper as a result of unsuitable soil found during excavations. Bash / Excavation Traveler #15 located at coordinates 4+35N and 3+63E identified an area under containment building Unit I that had unsuitable material that was required to be removed and replaced with Bash material. The placement of Bash lasted on the order of 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> to completely fill the over excavated area. This area is suspected to be the area of concern identified by the individual contacting the NRC resident. The records indicated that unsuitable material appeared to be properly identified and tested according to specification requirements and FSAR commitments.

Subsequent to researching the quality control records, the NRC inspector interviewed the concerned on-site individual. No further areas of concern were identified. The NRC inspector summarized the review and findings of the investigation.
