Areas Inspected: Unannounced inspection by one regional inspector of preoperational testing quality assurance activities. The inspection involved a total of 34 inspector-hours onsite.

Preoperational Testing Quality Assurance l
This inspection involved a review of the quality assurance program used for preoperational testing. Program implementation was also reviewes.

Compliance with regulatory requirements and quality assurance program
ccamitments was verified. The inspection was performed by observing work
activities, conducting personnel interviews and reviewing applicable i
procedures and records. Audits and surveillances of pre-test, testing and post-testing activities by the quality assurance organization were reviewed.
Test evaluations and reviews by the operations technical staff,
'
as well as methods used for documenting, tracking and resolving test
deficencies were also included in the inspection.

Preoperational testing for Byron Unit 2 is performed by systems test engineers (STEs) assigned to the operations technical staff. Pre-test
reviews and systems walk-downs are conducted at the time of system l
turnover to determine their status. This status is reviewed by STEs to i
determine system testability and the impact of incomplete or nonconforming

items on testing. Deficiencies noted prior to and during testing are documented on a deficiency form. Open and closed deficiency forms are included as a part of the test package. After testing is complete, the test is evaluated by the responsible STE.

include verification that the test is complete, test commitments have been met, retesting has been performed as required, and that open deficiencies are adequately docunented.
Deficiencies are tracked until appropriate action has been taken, the action has been verified and the deficiency closed.
The operations quality assurance organization performs either ardits or surveillances of test procedures prior to testing, selected portions of inprocess testing and completed test packages after testing is completed.
During this inspection the inspector reviewed several test packages to verify that accurate and appropriate reviews had been performed. Review checklists for both the TRB and the testing group leader were revicwed.
Records of preoperational testing audits and surveillances were reviewed, as well as the qualifications of the personnel performing the acdits and surveillances.
The inspector also irterviewed several systems test engineers and TRB members. Personnel interviewed appeared to be technically qualified and knowledgeable of preoperational testing requirements and the test review process. Records of preoperational testing audits and surveillances, and qualifications of personnel performing the audits and surveillances, were acceptable.
There were no violations or deviations noted.

Exit Meeting The inspector met with licensee representatives (denoted in paragraph 1)
at the Byron Plant on January 10, 1986, and summarized the purpose, scope ar.d findings of the inspection. The inspector discussed the likely informational content of the inspection report with regard to documents or processes reviewed by the inspector during the inspection. The licensee did not identify any such documents or processes as proprietary.
