Safety Evaluation Report (SER) Review (92719)

The inspector reviewed the status of SER Open Item 9, which concerns upgrading the pressurizer and steam generator power operated relief valves (PORVs) ~ to safety-related.

Reports 413/85-23 and 414/85-18. The Unit 2 pressurizer PORV modifications have been completed and preoperational (preop) testing is scheduled to be performed during Phase III of the hot functional testing (HFT).

Licensee personnel stated that the steam generator PORV modifications have not been completed.

The inspectors will continue to follow the progress of the modifications and testing during subsequent inspections.

Within the areas examined, no violations or deviations were identified.

Preoperational Test Procedure Review (70300, 70333, 70335, 70341, 70347, 70348)

The inspector reviewed the preoperational (preop) tests listed below to verify that they were consistent with applicable sections of FSAR Chapters 7, 8, 9, 10, and 14; SER and its supplements; Technical Specifications; Regulatory Guides 1.9, 1.41, 1.68, 1.79, and 1.108.

Preoperational (Preop) Test Witnessing (70441, 70433, 70312, 70435, 70438)

The inspectors witnessed portions of the following preop tests:

TP/2/A/1450/08A, Diesel Generator 2A Engine Starting Air Cold Functional Test, Sections 12.2 and 12.3, which verify the operation of staring' air compressors 2A1 and 2A2 to maintain adequate starting air pressure.

TP/2/A/1400/07A, Diesel Generator 2A Cooling Water System Cold

Functional Test (Sections 12.1 and 12.2) Section 12.1 verifies the proper operation of the cooling water's temperature, pressure, and level alarms.

Section 12.2 verifies the proper operation of the interlock between the Jacket Water Keep Warm Pump and the Jacket Water Keep Warm Heater.

. TP/2/A/1700/01A, Diesel Generator 2A Lube Oil System Cold Functional Test, Section 12.1, which verifies the proper operation of the Lube Oil's temperature, pressure, and level alarms.

TP/2/A/1100/02A, Diesel Generator 2A Preoperational Functional Test (Sections 13.5 and 13.11). Section 13.5 verifies the preper operation

of the engine trip devices during normal operation, which was run in conjunction with Section 13.11, which demonstrates that Diesel Generator 2A can be successfully started and loaded to 23500kw for 260 minutes.

TP/2/A/1200/05, Chemical and Volume Control Functional Test,

Section 12.1, which verifies the operability of the setpoints, alarms, and interlocks associated with the automatic letdown flow isolation resulting from low low pressurizer level.

-

TP/2/A/1250/04, Auxiliary Feedwater Functional Test (Sections 12.20 and 12.28).

Section 12.20 verifies that Auxiliary Feedwater Pump 2A will run successfully for 48 hrs. Section 12.28 verifies the absence of water hammer while feeding the steam generators with the auxiliary feedwater pumps through the auxiliary feedwater nozzles.

IP/0/A/3121/13, Pressurizer Safety Relief Valve Acoustic Monitoring

Device Functional Test and Calibration, portions of Section 10.0, which verifies the operability of the Valve Flow Monitoring System which provides an indication of relative valve position.

The above tests were witnessed to verify the following:

Hot Functional Testing (HFT) Procedure Review (70308)

The inspector reviewed test procedure TP/2/A/1100/01, Controlling Procedure for Hot Functional Testing, for consistency with FSAR Chapter 14, Regulatory Guide 1.68, and applicable vendor guidance.

The procedure was reviewed to verify that:

Hot Functional Testing Witnessing (70314)

The inspectors witnessed portions of test TP/2/A/1100/01 and related preop tests (which were part of HFT) IP/0/A/3121/13, TP/2/A/1200/05, and TP/2/A/1250/04. The tests were being performed at various temperature and pressure plateaus.

The related tests are also discussed in paragraph 7 above.
