Licensee Action on Previous Ins ection Findin s
(Open) Violation 50-410/84-08-01:
Failure to record and disposition minimum pipe wall thickness.
The inspector reviewed documentation related to licensee analysis of various piping welds which his investigation showed were below the code allowed minimum wall thickness, but which were
found, by analysis, to meet the design minimum wall thickness criteria.
Thickness measurements of weld number 09-6-2-WCS-008-861-FW006 indicated that a local'rea produced a reading which was below the minimum required design thickness.
A digital readout ultrasonic meter, similar to the equipment used to obtain the low reading, was used to re-measure the weld thickness and no readings were recorded which were below the minimum design thickness.
This item remains open pending the licensee's verifi-cation that the original thin area was included in the re-measurement data and that the weld does not violate the design minimum wall thickness.

Preservice Ins ection PSI Pro ram The preservice inspection program at Nine Mile Point, Unit 2 is intended to comply with the ASME Boiler and Pressure Vessel Code,Section XI, 1980 Edition through Winter 1980 Addenda.
That code edition requires that volumetric examination be done of the lower third of each weld supple-mented by a surface examination of the outside surface.
At the licensee's request the program at Unit 2 requires that volumetric examination be done of the entire weld as well as the code required surface examination.
The examinations are being done by Nuclear Energy Services personnel under the direction of Stone
& Webster Engineering. Company acting as the licensee's agent.
No violations were identified.

PSI Procedure Review The following nondestructive examination procedures were reviewed by the inspector with regard to technical adequacy, and to ascertain compliance with applicable ASME Code and regulatory requirements:

Observations of PSI in Pro ress The inspector observed the ultrasonic examination of the 28 inch diameter, Main Steam System weld 2-MSS-01-04-FW009.
The observation was made to ascertain that procedural requirements were met, and that applicable ASME Code and regulatory requirements were met.
The inspector found that the examination was done in accordance with procedure 80A7719 Revision 2, the examination equipment was currently calibrated, and the examination was performed by properly qualified personnel.
No violations were identified.

Review of PSI Data The inspector reviewed data associated with the following welds:

Main Steam System weld number MSS-Ol-17-SW004 The review was done to ascertain that findings were properly recorded and evaluated, and that ASME Code and regulatory requirements were met.
Indications which were identified by the ultrasonic method were recorded, plotted and dispositioned based on the plotted location of the reflectors.
Liquid penetrant.examination indications were recorded and Customer Notification Form number NMP2-031 was issued.
The examinations were performed by properly qualified personnel.
No violations were identified.
7.
Exit Interview The inspector met with licensee representatives (denoted in paragraph 1)
at the conclusion of the inspection on May 31, 1985.
The inspector summarized the purpose and the scope of the inspection and the findings.
At no time during this inspection was written material provided by the inspector to the license ~
