~8ack round On April 27, 1993, an open meeting was held at the Supply System Headquarters in Richland, Washington, with the individuals identified in paragraph 1.
The purpose of this meeting was to discuss the Systematic Assessment of Licensee Performance (SALP) for the period from January 1,
1992 to February 28, 1993, as presented in NRC Report No. 50-397/93-04.
The meeting convened at 8:00 a.m.
SALP Hang ement Neetin Hr. Faulkenberry opened the meeting by introducing the principal participants and by stating that during the NRC's presentation of the SALP areas, the licensee was encouraged to actively participate in dialogue between the NRC and the Supply System, so that an understanding of the most important issues could be conveyed.
Hr. Counsi 1 welcomed the NRC participants and complimented the NRC for the content of the report.
Hr. Johnson then summarized the overall strengths and weaknesses of the Operations section as discussed in the SALP report.
He concluded that the board had assessed the area as Category 2.
Hr. Partlow then discussed a
need for more aggressive leadership by the Operations Department in demonstrating ownership of the plant.
Hr. Parrish commented that he agreed that an improvement in teamwork is necessary in Operations, and that the Supply System will maintain close contacts with their industry counterparts to obtain the overall nuclear industry perspective on this subject.
Hr. Johnson then summarized the overall strengths and weaknesses in the Maintenance/Surveillance functional area, as discussed in the SALP report.
He concluded that the board had assessed this functional area Category 2,
with a declining trend.
Hr. Faulkenberry then expressed his concerns regarding repeat equipment failures, inadequate procedures, poor procedure compliance, and the lack of thorough forced outage planning.
Hr. Counsi 1 responded by stating that the Supply System had hired an outside consultant to aid them in developing effective corrective actions on these items.
Mr. Parrish stated that he recognized that performance had slipped in this functional area.
He added that the Supply System is performing a detailed review of the technical quality of surveillance procedures, as compared to the Technical Specifications.
In addition, Mr.
Parrish agreed that there was a large backlog of work and that it will be a challenging task to work down the numbers.
He also stated that there will be increased supervisory involvement and accountability to ensure procedure compliance.
Hr. Partlow commented that in initial and continuing training these messages must be communicated to and accepted by personnel at the working level for management's initiatives to be effectiv Hr. Johnson then summarized the overall strengths and weaknesses of the Engineering/Technical Support Functional area as discussed in the SALP report.
He noted that performance in this area appeared to have declined and concluded that the board had assessed this functional area Category 3.
Hr. Faulkenberry added that although the numerous problems found by the setpoint verification, design basis document, and other licensee programs were evidence of good engineering work, many of them should have been found much earlier.
He also stated that it was imperative for the Engineering staff to improve communications with management and Operations for the utility to improve.
Hr. Counsi 1 agreed with Mr. Faulkenberry's comments.
Hr. Wenslawski then summarized the overall strengths and weaknesses of the Radiological Controls functional area and stated that the board had assessed the area Category 2.
Nr. Partlow complimented the Supply System for the decrease in,contaminated floor space.
Nr. Parrish stated that the Supply System recognized the deficiencies in this functional area and has hired new people from good-performing utilities to help address these concerns.
Hr. Faulkenberry described the Supply System's performance in Radiation Protection as inconsistent.
He further stated that this was evidenced by good outage performance, balanced against lingering problems with the control of low level waste.
Mr. Wenslawski then discussed the overall strengths and weaknesses of the Emergency Preparedness (EP) functional area, as. outlined in the SALP report, and concluded that the board had assessed the area Category 2.
Hr. Parrish agreed that the EP area contained weaknesses in scenario development and staffing of the Technical Support Center, and stated that corrective actions were in progress t'o realize improvement.
Hr. Wenslawski then summarized the Security functional area, as discussed in the SALP report, and concluded that the board had assessed this area Category 1.
Mr. Faulkenberry commented that the Supply System was truly superior in this area.
Supply System management recognized the NRC's comments.
Nr. Perkins then discussed the Safety Assessment/guali ty Verification (SA/gV) functional area, as set forth in the SALP report, and concluded that the board assessed had this functional area Category 3, with an improving trend.
Hr. Partlow commented that facilities on the NRC's
"Watch List" had in common a Category 3 rating in SA/gV.
He pointed out that this area warranted particular attention if overall efforts to improve performance were to be effective.
Hr.
Roe added that the licensee's guality Assurance (gA) organization appeared to be competent in identifying issues, but was not being used effectively by Senior Management to ensure that issues identified are properly addresse Hr. Gearhart stated that the gA organization will strive to maintain their level of skill in identifying issues, and will work to improve corrective action tracking and escalation of findings.
Hr. Counsi 1 agreed with the NRC's assessment of SA/gV and the NRC's overall assessment of the Supply System.
He stated that his chief concerns were the maintenance and engineering backlogs, slow management response to issues, poor root cause analyses, inadequate work prioritiza-tion, the lack of teamwork, and inadequate teamwork within the utility.
He then committed to provide a response to all of the NRC's findings in the SALP report, complete with corrective actions that have an aggressive schedule for completion, and a trackable means to ensure closure.
Hr. Oxsen stated that part of the plan for improvement was to bring in new people for some senior management positions,'nd that the new Plant Manager and Engineering Director will be onsite in the near future.
Hr. Faulkenberry then closed the SALP discussion and solicited questions from the audience.
Hr. HcGi lton asked for further explanation of the SA/gV rating.
Mr. Perkins responded by stating that, although the NRC still saw repeat failures and slow or inadequate corrective actions, the influence of the new management team appeared to be bringing about a small amount of improvement in SA/gV.
Mr. Rhoads questioned the comments in the report about the adequacy of root cause analyses.
Hr. Perkins stated that the repeat equipment failures, recurrent personnel performance issues, and inadequate LERs were evidence that root cause analyses were not always critical and probing.
Hr. Johnson commented that the institution of Incident Review Boards was a
positive step in root cause identification.
Closin Remarks Hr. Faulkenberry concluded the meeting by stating the NRC had concerns with the Supply System's overall performance.
He explained that it appears that many performance problems are evident throughout the Supply System, and have been around for many years.
He added that these problems will require significant management effort to realize long-term improvement.
He then thanked the licensee for their participation, and adjourned the meeting at 9:27 ~
