This violation (issued on December 28, 1992)
involved the removal of servic6 air check valves designed for permanent installation without the required approvals, engineering evaluations, or required documentation.
A similar problem was identified on Hay 5, 1993, by licensee Problem Evaluation Request (PER) 293-489.
This PER identified that service air check valve SA-V-102/7 had been removed without the required approvals and required documentation.
The inspectors verified that every service air line identified during daily tours of the Turbine Building, Radwaste Building, and the Reactor Building had a check valve installed with an attached white label identifying it as a service air check valve with the words "S.A.
CHECK VALVE [-) DO NOT REMOVE [-] CONTACT SHIFT MANAGER."
