Dear Edward Pigott:

On September 14, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at McGuire Nuclear Station and discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.

One finding of very low safety significance (Green) is documented in this report. This finding involved a violation of NRC requirements. We are treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2 of the Enforcement Policy.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an Age Related Degradation Inspection at McGuire Nuclear Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. The NRC identified a Green Finding and associated Non-cited Violation (NCV) of 10 CFR 50,

Appendix B, Criterion XI, Test Control, for the licensee's failure to incorporate the hose bend radius acceptance limits as provided by the hose vendor into the test procedure used to verify the correct installation of the fuel oil and lube oil hoses on Emergency Diesel Generators (EDGs) 1A, 2A, 1B, and 2B.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

REACTOR SAFETY

71111.21N.04 - Age-Related Degradation Age-Related Focused Engineering Inspection (section 03.01)

For each sample listed below, inspectors reviewed licensee activities credited to address age-related degradation to assess whether they were being performed using appropriate standards and procedures, assessed against appropriate acceptance criteria, and that maintenance was being performed at an appropriate interval. Inspectors assessed whether issues identified during licensee activities were being appropriately entered into and addressed by the station corrective action program, and whether periodic assessments of maintenance effectiveness and operating experience were being completed.

INSPECTION RESULTS

The NRC identified a Green Finding and associated Non-cited Violation (NCV) of 10 CFR 50, Appendix B, Criterion XI, Test Control, for the licensee's failure to incorporate the hose bend radius acceptance limits as provided by the hose vendor into the test procedure used to verify the correct installation of the fuel oil and lube oil hoses on Emergency Diesel Generators (EDGs) 1A, 2A, 1B, and 2B.

Description:

During a walkdown of the EDGs, the inspectors identified that the flexible hoses installed on EDGs 1A, 2A, 1B and 2B used to deliver fuel oil and lube oil to the engines had bend radii smaller than the minimum bend radius published by the hose manufacturer, Eaton Aeroquip. The fuel oil injection pump suction/supply hoses (Catalog ID 864263 and Nordberg P/N 4425 3884) on the emergency diesel generators are Aeroquip FC300-10 material.

Aeroquip literature specifies a minimum bend radius of 5.5 inches for this hose material and diameter. The lube oil supply to the link shaft actuator is through a flexible hose (Catalog ID 864234 and Nordberg P/N 4425 4051) made from Aeroquip FC300-12 material. Aeroquip literature specifies a minimum bend radius of 6.5 inches for this hose material and diameter.

In response to the observation, the licensee initiated NCR 02486130, conducted an operability determination, and determined the EDGs remained operable. This determination was based, in part, on the licensee's prior evaluation to extend the hose replacement interval from six to 12 years. For this evaluation, the licensee removed a set of used hoses from the 1A diesel generator in 2020 and sent them for testing. The vendor lab tested new and used hoses to quantify the changes in mechanical properties of hoses that had been installed on 1A diesel for 5.6 years. While the hoses were not tested with the bend radius as installed on 1A EDG, the licensee determined the vendor testing supported the conclusion that current and near-term operability was not impacted by the configuration.

During the extent of condition review, the licensee determined 56 fuel oil hoses had a bend radius smaller than the manufacturer's acceptance limits, including 16 fuel oil hoses on EDGs 1A, 2A, and 2B and 8 fuel oil hoses on the left bank cylinders of EDG 1B. The licensee also determined that the lube oil hoses on EDGs 1A, 1B, and 2A had a smaller bend radius than specified by the hose manufacturer. There were no signs of leakage or damage to the fuel and lube oil hoses at the time of discovery, however, hoses with tighter bends may potentially experience abnormal wear and fatigue resulting in possible premature failure and downstream fluid pressures and/or flows inside the hose to be outside acceptable parameters.

In 2006, as part of the stations change in hose suppliers from original parts supplier, NAK Engineering, to the commercial hose manufacturer, Aeroquip, the licensee conducted a reverse engineering evaluation documented in Duke Energy Company Reverse Engineering Technical Document No. REDI 2020.00-00-0005, Aeroquip Fuel and Oil Hoses for MNS Diesels." The inspectors reviewed REDI 2020.00-00-0005 and test procedure PT/0/A/4350/030, "Nordberg Diesel Engine 6 Year Periodic Maintenance," Revisions 023, 027 and 029, which the licensee used to replace the hoses and accept the installation and determined that bend radius acceptance limits provided in REDI 2020.00-00-0005 were not translated into applicable sections of the procedure. The licensee previously installed the hoses under Work Orders -20378334-13, -20257780-02, -02102410-02, -20464037-01, 20464038-07, and 20464038-02, on the 1A EDG (June 2020 internal and external hose replacement), the 1B EDG (April 2019 internal and external hose replacement), the 2A EDG (April 2014 external hose replacement), the 2A EDG (July 2021 internal hose replacement),the 2B EDG (July 2022 external hose replacement), and the 2B EDG (March 2023 internal hose replacement), respectively.

Corrective Actions: The licensee initiated a condition report, conducted an operability determination, and determined the EDGs remained operable. Additionally, the licensee initiated Work Requests to replace the existing lube and fuel oil hoses on all four EDGs so the minimum bend radii is not exceeded.

Performance Assessment:

Performance Deficiency: The installation procedure for the EDG fuel oil and lube oil hoses (PT/0/A/4350/030) did not contain acceptance limits in document (Duke Energy Company Reverse Engineering Technical Document REDI Number 2020.00-00-0005, Aeroquip Fuel and Oil Hoses for MNS Diesels) for bend radius as provided by the hose vendor. The Duke Quality Assurance Program Document for Catawba, McGuire and Oconee, section D17.3.2.8, Test Control, required that:

Test procedures contain the following information or require this information be documented: 1) Requirements and acceptance limits contained in applicable design and vendor documents.

Screening: The inspectors determined the performance deficiency was more than minor because it was associated with the Equipment Performance attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the inspectors determined the assurance of the reliability of the hose was impacted by the smaller than specified bend radius.

Significance: The inspectors assessed the significance of the finding using IMC 0609 Appendix A, The Significance Determination Process (SDP) for Findings At-Power. Exhibit 2 - Mitigating Systems Screening Questions, Item A. Mitigating SSCs and PRA Functionality (except Reactivity Control Systems), which specified that if the finding affected the design or qualification of a mitigating SSC, but it maintained its operability or PRA functionality then it screens to GREEN.

Cross-Cutting Aspect: Not Present Performance. No cross-cutting aspect was assigned to this finding because the inspectors determined the finding did not reflect present licensee performance.

Enforcement:

Violation: 10 CFR 50, Appendix B, Criterion XI, Test Control, requires, in part, that A test program shall be established to assure that all testing required to demonstrate that structures, systems, and components will perform satisfactorily in service is identified and performed in accordance with written test procedures which incorporate the requirements and acceptance limits contained in applicable design documents.

Contrary to the above, the test program for the EDG, which included PT/0/A/4350/030, did not incorporate requirements and acceptance limits contained in applicable design documents. Specifically, limits on the fuel oil and lube oil hose bend radius contained in applicable vendor documents contained in REDI Number 2020.00-00-0005 were not incorporated into PT/0/A/4350/030.

Enforcement Action: This violation is being treated as a non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.
