Areas Inspected:
Included a review of a security incident involving a Vital Area (VA) barrier degradation that occurred between March 4 and 5,1986.
Results: Based on this inspection, two apparent violations were identified:
1.
Access Control - Personnel: The licensee failed to adequately control access to a vital area.
2.
Records and Reports: The licensee failed to report to the NRC a major loss of security effectiveness, uncompensated for, within the one hour requirement specified in 10 CFR 73.71(c). The event was reported late.
