Inspection Summary Inspection on March 5-24, 1980 (Report No. 50-346/80-09)

Areas Inspected: A special announced inspection to verify the licensee compliance with Item 1, a, b, and c of an Order to Modify the Licensee issued March 5,1980 to upgrade the experience level of nonlicensed members of the operating shif ts.

The inspection involved 30 inspec-tor-hours onsite by the resident inspector.

Results: No items of noncompliance or deviations were identified.

Background

'

A recent evaluation of the licensee's operation showed that despite efforts of the licensee to increase the staffing levels of the plant, it had not reached the desired level. Known turnovers, transfers and operator training had resulted in the use of overtime to maintain the operating shift manning levels.

In February 1980, a letter was sent from the plant reactor opera-tors to the Vice President, Nuclear, expressing concern about the loss of qualified operators and requesting a meeting on February 17, 1980.

While this letter was directed towards correcting the reason why reactor operators were leaving, it contained an implied concern about the present status.

During the week of February 18, 1980, the Resident Inspector inter-viewed plant operators to determine the extent of the concerns expressed in their letter to the Vice President. Under direct questioning the Resident Inspector was told by one operator that the staffing problems were so significant that the plant was unsafe to operate.

On February 25, 1980, the Director of Region III and the Director, Division of Reactor Operations Inspection, IE interviewed plant operators.

Perceived problems of excessive work hours and inade-quate training were identified during these interviews. Some of those interviewed, including two senior licensed operators stated that in their view some shifts did not have adequately qualified people.to successfully handle an emergency. None of those inter-viewed rated the plant as being any better than marginally safe.

Additional investigation was conducted onsite on February 26 and l

27, 1980. These investigations showed that while the licensee

meets the staffing requirements of its license, concerns remain

'

regarding the adequacy of training of nonlicensed personnel,.the ability to respond to emergency conditions, and the overall depth of the staff. Specifically:

a.

Six of the more experienced eqt.ipment operators had been effectively removed from regular shift work to train to become licensed operators; b.

Some of the remaining operators have been trained on the

'

equipmeat only in certain zones. While this is adequate for routine shift operations, these equipment operators are not trained on all safety related equipment such that they could be relied upon if needed in other work zones during an emer-gency.

c.

There was little flexibility to accommodate the loss of staff because of sickness or other work leave. Such problems result in additional overtime work requirements.

These concerns were addressed with representatives of the licensee during a management meeting on February 29, 1980 in which the licensee presented a corrective action program (Inspection Report No. 50-346/80-07). The corrective action was the subject of an Order modifying the license on March 5, 1980.

The Order required the licensee to immediately:

a.

Return six experienced and fully qualified equipment opera-tors, who have been involved in operator license training, to regular shift work to provide two fully qualified equipment operators for each operating shift. The additional qualified individuals will remain on shift duty until such time as they are relieved from duty by other fully qualified individuals who are appropriatelty trained and certified by the licensee's training department.

b.

Provide one additonal person for each day shift to relieve the shift foreman of nonlicensed administrative responsibilities.

c.

Expedite the on-the-job training schedule for existing equipment operators who are not yet fully qualified. This program shall be accomplished by personnel from Toledo Edison Company or an outside contract organization and shall continue at least until the refueling outage.

Or in the alternative to items a, b, and c, above, place and maintain the facility in a cold shutdown or refueling mode of operation.

t The order also required that:

Prior to return of the station to operations from the scheduled April 1980 refueling outage, each operating shift shall have a complete complement of fully trained personnel in accordance wi?.h the staffing and qualification levels set forth in draft ANSI (ANS)

Standard 3.1.

Immediate Requirements Ite: a.

The inspector verified that the six fully qualified equipment operators who had been involved in operator license training were immediately returned to regular shift work to provide two fully qualified equipment operators for each operating shift. The in-spector also verified that the qualified equipment operators were relieved of their shift duty only when other fully qualified in-dividuals were available to fulfill shift manning requirements.

The inspector verified that all the fully qualified equipment

, operators were certified by the licensee's training department.

Item b The inspector verified that the licensee has provided one addi-tional persons for each day shift and that his duties are to relieve the shift foreman of nonlicensed administrative responsi-bilities.

The inspection of items a. and b. above consisted of observation of all five shifts while on duty, examination of the Reactor Operator and Shift Foreman Log and interviews with licensee personnel.

Item c.

The inspector verified that the training schedule of equipment operators not fully qualified was expedited. The inspector attended the classroom training on March 11, 13, 1980 and the oral examinations given to four operators while completing the systems walkdowns on March 14, 1980.

The inspector reviewed the qualification exam given to the equipment operators and determined that it contained indepth questions on all safety related systems for normal and abnormal operating conditions. The inspector also verified that only those persons who completed the training with a grade of 80%

~ or higher were certified as qualified equipment operators.

5.

Exit Interview The inspector met with licensee representatives (denoted in Paragraph 1) throughout the inspection and summarized the scope and findings of the inspection activities.
