L-89-440AttachmentVIOLATIONA.Unit1TechnicalSpecification6.8.l.a.requiresthatwrittenproceduresshallbeestablished,implemented,andmaintained,'coveringtheactivitiesrecommendedinAppendix"A"ofRegulatoryGuide1.33,revision2,February1978.Appendix"A",paragraph7.a.,recommendsproceduresfortheliquidradioactivewastesystemincluding"demineralizing".Appendix"A"paragraph3.n.,recommendsproceduresforthechemicalandvolumecontrolsystem,whichcontainssomeoftheionexchangersaddressedinthesubjectprocedure.Thesewereimplementedin-partonsitebyoperatingprocedureOP1-0520020,RadioactiveResinReplacement,whichprovidedinstructionsforresinreplacementonvariousionexchangers,includingthe1Dwasteionexchanger.Contrarytotheabove,onAugust20,1989,thelicenseefailedtoimplementtheaboveprocedure.Theprocedurewassignedasbeingcompletefor.the1Dwasteionexchanger;however,theresinfillfunnelwasleftinstalledontheresinfillisolationvalveflangeandexposedtotheweatheruntilSeptember8,1989,whenitwasdiscoveredbyanNRCinspector.Thefillisolationvalve,whichwasapotentialunmonitoredradioactivereleasepoint,islocatedinashallowtrenchontheauxiliarybuildingroof.Thisfillstation,asshownbydiagramsintheFinalSafetyAnalysesReport,wouldnormallybesealedafterresinfillingbyaboltedandgasketedblankflangeandwouldalsobeprotectedbyaboltedmetaltrenchcover.Subsequentprocedurereviewshowedthattheprocedurewasalsoinadequatelyestablished.Itdidnotadequatelyaddressfunnelinstallation/removal;blankflangeremoval,temporarystorage,orreinstallation;stowageinstructionsforthepotentiallycontaminatedfunnel;nordiditprovideadequatemanagementcontrolofdepartmentalinterfacesduringtheprocess.RESPONSEA:1~FloridaPower&Lightconcurswiththeviolation.2~Thecauseoftheviolationwaspersonnelerrorandinadequateprocedure.Theprocedurebeingimplementeddidnotprovidethespecificactionstobeaccomplishedwhenresinreplacementwascomplete.3~4~Theresinfillfunnelswereremoved,gasketedflangesinstalledandthemetaltrenchcoverswerereplaced.OperatingProcedures1-0520020and2-0520020willberevisedto'rovidetheinterdepartmentalcontrolsfortheinstallationandremovaloftheresinfillfunnelsandblankflanges.5.CorrectiveactionsarescheduledtobecompletedbyFebruary1,199 L-89-440AttachmentPg2VIOLATIONB:Units1and2TS6.8.1arequirethatwrittenproceduresshallbeestablished,implemented,andmaintainedcoveringtheactivitiesrecommendedinAppendix"A"ofRegulatoryGuide1.33,revision2,February1978.Appendix"A",paragraph9.aincludesproceduresformaintenanceofsafety-relatedequipment.Thisrequirementwasimplementedinpartbyprocedures1-M-0018and2M0018)MechanicalMaintenanceSafety-RelatedPreventiveMaintenance(PM).Program,thatimplementedPMrequirementsonthestrainersforthebearinglubricatingwaterfortheUnit1and2intakecoolingwater(ICW)pumps.Contrarytotheabove,thesubjectprocedureswereinadequatelyestablishedinthattheydidnotprovideappropriateinspectionguidanceconcerningself-cleaningstrainersandy-typestrainersinthebearinglubricatingwatersystemsfortheICWpumps.Theproceduresdidnotprovidestrainerinspectionacceptancecriteria,servicelimits,orrepairspecifications.Thelackofinspectionguidanceallowedachangeinstrainermeshsizetogounquestionedandexcessivedeteriorationofself-cleaningstrainerstobeunrecognized.ThechangeinstrainermeshsizewasamajorcontributortolAICWpumpbearingfailurescausedbyy-typestrainerblockage.RESPONSEB'loridaPower&Lightconcurswiththeviolation.2~Thecauseoftheviolationwasaninadequateprocedure.Theinspectionacceptance.criteria,servicelimits,andsparepartsinformationwerenotprovidedin'theICWpumpbearinglubricatingwatersystempreventativemaintenanceactivities.(PMfilenumbers153,154,155,and156forUnit1andPMfilenumbers00501,00502,00503,and00504forUnit2).TheseactivitiesarespecifiedintheMechanicalMaintenanceSafety-RelatedPreventive,MaintenanceProgramProcedures(1-M-0018and2-M-0018).3~Thepreventativemaintenance(PM)activitiesfortheICWpumpbearinglubricatingwatersystemspecifiedinprocedures1-M-0018and2-M-0018havebeen'evisedtoprovidetheinspectionacceptancecriteria,repairguidelinesandsparepartsinformatio L-89-440AttachmentPg34ThemaintenancedepartmentwillreviewarepresentativesampleofotherPMsinprocedures1-M-0018and2-M-0018todetermineif'heinspectionacceptancecriteria,repairguidelinesandsparepartsinformationareincluded..Basedon.theresultsofthisreviewthecorrectiveactionsspecifiedinitem,3abovewillbeexpandedtoincludeadditionalPMactivitiesaswarranted.ThisreviewisscheduledtobecompletedbyJanuary31,1990.5.Item3abovewascompletedonOctober30,198 P
