SUBJECT: NRC INSPECTION REPORT NOS. 50-327/38-06 AND 50-328/88-06 Thank you for your response of March 30, 1988, to our Notice of Violation issued on March 1, ~ 1988, concerning activities conducted at your Sequoyah facility. With regard to your response to violation 88-06-01, your reason for the violation appears to contradict information contained in inspection report-327, 328/87-66. As discussed in inspection report 327, 328/87-66, paragraph 3d, a meeting was held during the October 19-30, 1987 inspection to discuss the issue of qualification for personnel involved in the system alignment process.
.The - meeting was attended by the Operations Superintendent, Operations Group Manager, and other management involved in the system alignment program.
Your staff stated that being a TVA auxiliary unit operator (AV0) along with having completed site specific training on all Sequoyah procedures pertaining to system alignment was considered the minimum acceptable qualification level _ for individuals performing independent verification for the system alignment program. In addition, the inspectors pointed out that since Sequoyah was using personnel other than those certified at Sequoyah, some documentation of the certification was necessary. Your staff accomplished this prior to the exit on October 30, 1987. Your response appears to have not taken these discussions into account and therefore does not fully address the reason for the violation.
Please provide a supplemental response within 30 days of the date of this letter to clarify the reason for the violation.
With regard to your response to violation 88-06-03, your corrective steps that have been taken appear to contradict information presented by your staff during the inspection, and summarized in the inspection report. The results of your inspection of the 1E electrical panels completed on January 16, 1988, were reviewed during the inspection and summarized in paragraph 6.b.(5) of the report.
This summary indicates that as many as six loose items were not removed due to their close proximity, to power. While your response indicates that all were removed during the January 16, 1988 inspection. Please provide a supplemental response within 30 days of the date of this letter which clarifies whether the additional six items have now been removed.
